目的 观察Notch1受体在胃癌患者中的表达,并分析其临床意义.方法 选取2013年2月至2015年2月在四川绵阳四〇四医院接受治疗的150例胃癌患者为胃癌组,选取同期检查确诊的150例萎缩性胃炎患者作为对照,比较两组患者胃黏膜组织中Notch1受体阳性表达率以及不同临床病理特征胃癌患者Notch1受体阳性表达率的差异,随访2年,观察死亡组和存活组胃癌患者Notch1受体阳性表达率.结果 胃癌组患者组织中Notch1受体阳性表达率为70.0%,高于萎缩性胃炎组的13.3%,差异有显著统计学意义(P<0.01);分化程度为中低分化、有淋巴结转移,TNM分期为Ⅲ+Ⅳ期的胃癌患者的Notch1受体阳性表达率较高(P<0.05);回归分析结果显示,分化程度和TNM分期是影响胃癌患者Notch1受体阳性表达率的因素(P<0.05);死亡组患者的Notch1受体阳性表达率为100.0%,明显高于存活组的65.9%,差异有显著统计学意义(P<0.01).结论 胃癌患者的Notch1受体阳性表达率较高,其与患者的肿瘤分化程度、TNM分期和患者预后密切相关.%Objective To observe the expression of Notch1 receptor in gastric carcinoma, and analyze its clinical significance.Methods A total of 150 cases of patients with gastric cancer who treated in Sichuan Mianyang 404 Hospital from February 2013 to February 2015 were selected as gastric cancer group, and 150 cases of patients with atrophic gastritis diagnosed during the same period were selected as control group.The positive rates of Notch1 receptor expression in gastric mucosa were compared between the two groups,and also between gastric cancer patients with different clinicopathological characteristics.With a follow-up of two years,the positive expression rate of Notch1 receptor in gastric cancer patients in the death group and survival group were observed.Results The positive expres-sion rate of Notch1 receptor in the gastric cancer group was 70.0%,which was significantly higher than 13.3% in the atrophic gastritis group(P<0.01).The positive expression rate of Notch1 receptor was higher in patients with medium and low differentiation, with lymph node metastasis, and with TNM stage Ⅲ+Ⅳ (P<0.05). The regression analysis showed that the degree of differentiation and TNM staging were the factors affecting the positive expression of Notch1 receptor in gastric cancer patients (P<0.05). The positive expression rate of Notch1 receptor in the death group was 100.0%, which was significantly higher than 65.9% in the survival group (P<0.01). Conclusion The expression of Notch1 receptor is high in patients with gastric cancer, and is closely related to the degree of differentiation, TNM stage and prognosis.
