贲门胃底癌经腹手术的两种不同手术方式比较

摘要

Objective To investigate the efifcacy of the treatment of total stomach resection and proximal stomach resection through abdominal approach for cardia adenocarcinoma.Methods 106 patients with cardia adenocarcinoma were divided into 2 groups according to the operation method in our hospital.56 patients were performed by total gastric resection and the other group were 50.Result Intraoperative conditions, postoperative complications and survival rate of the two groups patients were compared, there are no signiifcant statistical differences (P>0.05) between the two groups in intraoperative bleeding, operation time . But signiifcantly less than the latter in the postoperative complications (P<0.05) and there are no significant statistical differences in the postoperative survival rate (P>0.05). Conclusion Total stomach resection through abdominal approach for cardia adenocarcinoma have no obvious difference in intraoperative bleeding, operation time, survival rate, but have obvious advantages in hpostoperative complications compare with proximal stomach resection.%目的：探讨贲门癌经腹全胃切除和近端胃切除手术治疗的疗效比较。方法回顾我院确诊的106例贲门胃底癌的患者根据手术方式分为2组，经腹全胃切除组56例，经腹近端胃切除组50例。结果对两组患者的术中情况、术后并发症及术后的生存率进行比较，经腹全胃切除组与经腹近端胃切除组在术中出血、手术时间方面无明显的统计学差异（P＞0.05），在术后并发症明显少于后者（P＜0.05），术后生存率无明显统计学差异（P＞0.05）。结论经腹全胃切除组与经腹近端胃切除组在术中出血、手术时间及术后生存率无明显的差异，但在术后并发症有明显的优势。
