宫颈细胞癌及宫颈上皮内肿瘤组织中HPV16感染与hTERT、Ki67表达的关系

         

摘要

目的:研究宫颈鳞状细胞癌与宫颈上皮内肿瘤组织中HPV16感染情况与hTERT和Ki67在组织中表达的关系。方法以2008年1月至2013年10月在我院进行治疗的73例妇科肿瘤患者和同期内在我院进行常规妇科检查的35例正常患者的宫颈组织为研究标本。研究采用免疫组化技术,组织芯片技术以及原位杂交方法等。结果病理宫颈组织(除CINⅠ以外)中检测出的HPV16阳性率均比正常宫颈组织高(P<0.05)。病变宫颈组织中CINⅡ、CINⅢ、原位癌、宫颈浸润鳞癌的hTERT表达以及Ki67表达均高于正常宫颈组织(P<0.05)。浸润鳞癌组织中的hTERT表达量显著高于原位癌组织中的hTERT表达量(P<0.01)。浸润鳞癌、原位癌的Ki67表达量比正常宫颈组织中的Ki67表达量检测值高(P<0.05)。HPV16阳性患者hTERT表达显著高于HPV16阴性患者(P<0.01),且HPV16感染情况与hTERT表达情况呈显著正相关关系(r=0.372)。HPV16感染情况与Ki67表达也存在统计学意义(P<0.05),而HPV16感染情况与Ki67表达无相关性(r=0.092)。此外,在病理宫颈组织中hTERT表达与Ki67表达有统计学意义(P<0.05),但存在显著正相关关系(r=0.396)。结论HPV16感染可能导致CIN的发生并逐步发展为SCC,而HPV16感染后组织中hTERT、ki67表达会发生改变,通过hTERT、ki67指标检测有助于宫颈癌癌前发现,对治疗和判断预后也有一定的作用。%Objective The relationship between HPV16 infection and expression of hTERT and Ki67 in the tissue of cervical squamous cell carcinoma and cervical intraepithelial neoplasia tissues. Methods From 2008 January-2013 year in October for routine gynecological examination in our hospital for treatment of 73 cases of gynecological cancer patients and the same period in our hospital 35 cases of normal cervical tissue specimens of patients. Study used immunohistochemical techniques, tissue microarray and in situ hybridization method. Results The pathological cervical tissues (except CINⅠ outside) the positive rate of HPV16 was detected in normal cervical tissue were higher than high (P<0.05). Higher than normal cervical tissue hTERT expression of CIN II, CIN III, carcinoma in situ, invasive cervical squamous cell carcinoma and the expression of Ki67 in cervical lesion tissues(P<0.05). Invasivesquamous cell carcinoma hTERT expression was signiifcantly higher than that of in situ carcinoma hTERT expression(P<0.01). Carcinoma in situ squamous cell carcinoma, Ki67 expression detection value higher than normal cervicaltissue Ki67 (P<0.05). Expression of HPV16 in hTERT positive patients was significantly higher than that in HPV16 negative patients (P<0.01), and the status of HPV16 infection and the expression of hTERT was signiifcant lypositive correlation (r=0.372). There are also signiifcant HPV16 infection and the expression of Ki67 (P<0.05), but there was no correlation between the expression status of HPV16 infection and Ki67 (r=0.092). In addition, in pathological cervical tissue hTERT expression and Ki67 expression was statistically signiifcant(P<0.05), but there was a signiifcant positive correlation between (r=0.396). Conclusion HPV16 infection may lead to the occurrence of CIN and the progressive development of SCC, hTERT, Ki67 expression change of HPV16 infected tissues by hTERT, Ki67, index detection helps to cervical precancerous found, also has a certain effect on the treatment and prognosis.
