超声引导行右颈内静脉穿刺置管术的应用

摘要

Objective To explore the clinical application of ultrasound guided percutaneous right internal jugular vein catheterization in hemodialysis, and to analyze the methods,precautions and prevention of complications of puncture.Methods Totally 165 patients of chronic renal insufficiency with uremia underwent ultrasound guided percutaneous right internal jugular vein catheterization according to technical points of increasing the success rate of puncture.Results The total successful rate of puncture was 100%.The first puncture succeeded in 148 patients(148/165,89.70%),the second in 9 patients (9/165,5.45%)and 8 patients (8/165,4.85%).The complication rate was 7.88%(13/165). Compression hemostasis was effective method. Conclusion Ultrasound guidance can greatly improve the success rate of puncture in percutaneous right internal jugular vein catheterization, while reduce the incidence of complications.%目的：探讨超声引导下行右颈内静脉穿刺置管术的应用价值。方法回顾分析成都铁路分局医院肾内科2010年1月至2013年6月共165例尿毒症患者，超声引导下经皮右颈内静脉穿刺置管术建立临时血管通路的成功率及并发症的预防。结果①第一针穿刺成功率89.70%，第二针穿刺成功率5.45%，未穿刺患者占4.85%。②渗血发生率7.88%，均压迫后止血。结论超声引导下行右颈内静脉穿刺置管术，极大提高置管的成功率，有效降低并发症的发生。
