纤维支气管镜下支气管肺泡灌洗联合局部注药治疗重症肺炎的Meta分析

摘要

目的 评价纤维支气管镜支气管肺泡灌洗联合局部注药治疗重症肺炎的效果.方法 计算机检索中国期刊全文数据库、维普数据库、万方数据库、临床决策循证数据库、Medline、Cochrane、PubMed,纳入有关纤维支气管镜肺泡灌洗联合局部注药治疗重症肺炎的随机对照试验(RCT),对纳入文献进行质量评价,应用RevMan5.3软件进行Meta分析.结果 共纳入30篇RCT,共2300例患者.Meta分析结果显示,观察组的治疗总有效率高于对照组,而住院时间及抗生素使用时间短于对照组(均P＜0.05).结论 在常规治疗的基础上,行纤维支气管镜下支气管肺泡灌洗联合局部注药能提高重症肺炎的疗效.%Objective To evaluate the efficacy of bronchoalveolar lavage combined with local irrigation under fiberoptic bronchoscope for severe pneumonia.Methods The databases of computer retrieve included CNKI,VIP,Wanfang database,FEBM,Medline,Cochrane and PubMed.Randomized controlled trials(RCTs) about bronchoalveolar lavage combined with local irrigation under fiberoptic bronchoscope for severe pneumonia were collected.The quality assessment of the studies included was performed,then Meta-analysis was conducted using RevMan 5.3 software.Results Thirty RCTs were involved,which consisted of 2 300 patients.The results of Meta-analysis showed that the total effective rate was higher and the hospital stay and duration of antibiotic use were shorter in the observation group compared to the control group(all P＜0.05).Conclusion Based on the conventional therapy,bronchoalveolar lavage combined with local irrigation under fiberoptic bronchoscope can improve the efficacy on severe pneumonia.
