特发性早产与白细胞介素-6及C-反应蛋白的关系的研究

摘要

Objective To study the pathogeny and mechanism of idiopathic premature labor. Methods The ma-ternal serum and vaginal secretion from the preterm labor (PTL) group, term labor (TL) group and control group were obtained for measurement of C-reactive protein (CRP) and Interleukin-6(IL-6) concentration. Results There was no difference of the serum levels of CRP among three groups (P>0.05). The concentration of CRP in the vaginal secretions in PTL group was higher than that of TL group and control group (0.240g/L vs 0.156 g/L and 0.068g/L, P<0.05). There was no difference of CRP levels in the vaginal secretion between TL group and control group(P>0.05). The concentration of IL-6 was elevated markedly in both vaginal secretion (0.481ng/ml vs 0.262ng/ml and 0.180ng/ml,P<0.05) and maternal serum (0.399ng/ml vs 0.204 ng/ml and 0.122ng/ml, P<0.05) of patients in PTL group than those of TL group and control group. There was no difference of IL-6 levels in both the maternal serum and the vaginal secretions between TL group and control group. There was significant positive correlation between IL-6 concentration in maternal serum and in vaginal secretion(y=0.513, P<0.05) in the pa-tients of PTL group. Conclusions Both the indexes could be used to predict the outcome of premature labor. E-valuating maternal serum IL-6 is more sensitive and convenient than CRP for premature labor.%目的 探讨特发性早产的病因及发病机理.方法 分别测定并比较了特发性早产患者、正常晚孕孕妇和正常足月分娩孕妇外周血和阴道分泌物中的C-反应蛋白(CRP)和白细胞介素(IL-6)的浓度.结果 特发性早产患者母体外周血中CRP浓度与正常晚孕孕妇和正常足月分娩孕妇无明显差异(P>0.05),而阴道分泌物中CRP浓度明显升高f0.240g/L,0.156 g/L和0.068g/L,P<0.05);正常晚孕孕妇和正常足月分娩孕妇的阴道分泌物CRP浓度无显著性差异(P>0.05).特发性早产患者母体外周血(0.399ng/ml,0.204ng/ml和0.122ng/ml,P<0.05)及阴道分泌物中(0.481ng/ml.0.262ng/ml和0.180ng/ml,P<0.05)的IL-6浓度明显高于正常晚孕孕妇和正常足月分娩孕妇,正常晚孕孕妇和正常足月分娩孕妇的外周血和阴道分泌物的IL-6浓度无显著性差异.特发性早产组的母体外周血和阴道分泌物的IL-6浓度呈明显正相关性(y=0.513,P<0.05).结论 特发性早产可能是亚临床感染所致,母体CRP和IL-6含量与特发性早产密切相关,可被用来预测特发性早产.检测母体外周血IL-6预测特发性早产更为敏感和方便.
