目的:了解广东省第二中医院 (以下简称"我院") 中药注射剂的临床使用情况, 为促进中药注射剂的合理应用提供参考.方法:利用医院信息系统, 随机抽查2016年6月至2018年6月我院住院归档病历, 每月随机抽取1个病区当月归档病历50份, 共抽取病历1 200份, 对我院24个临床病区28个品种34个品规的中药注射剂使用情况进行回顾性点评与分析.结果:1 200份住院病历中, 使用中药注射剂的病历有826份;其中, 合理使用中药注射剂的病历260份, 用药合理率为31.48％ (260/826).中药注射剂在我院临床各科室的使用率均较高, 但用药合理率较低, 不合理用药主要涉及活血化瘀类、补益类中药注射剂;不合理用药主要表现为适应证不适宜、用法与用量不适宜、溶剂选择及剂量不适宜和联合用药不适宜.结论:中药注射剂在我院的应用日趋广泛, 不合理用药现象比较严重, 将采取综合干预措施, 逐步规范中药注射剂的临床应用, 提高医疗服务质量, 保证患者用药安全.%OBJECTIVE: To investigate the clinical use status of traditional Chinese medicine (TCM) injections in Guang Dong the Second Traditional Chinese Medicine Hospital (hereinafter referred to as "our hospital"), so as to provide reference for promoting the rational application of TCM injections. METHODS: The hospital information system was used, the archived medical records in our hospital were randomly selected from Jun. 2016 to Jun. 2018. And 50 archived medical records were randomly selected from one ward every month, and a total of 1 200 medical records were selected. The use of TCM injections of 28 varieties and 34 regulations in 24 clinical wards of our hospital was reviewed and analyzed retrospectively. RESULTS: Of the 1 200 inpatient medical records, 826 were recorded using TCM injections, among which, 260 cases of rational use of TCM injections were recorded, and the rational rate was 31.48％ (260/826). The use rate of TCM injections in all clinical departments of our hospital was relatively high, but the rational rate of drug use was low. The irrational use of drugs mainly involved the injection of invigorating blood circulation and eliminating stasis and tonic drugs. The irrational use of drugs mainly performed as inappropriate indications, improper usage and dosage, irrational solvent selection, inappropriate dosage form and irrational drug combination. CONCLUSIONS: The use of TCM injections in our hospital is becoming more and more widespread, yet the phenomenon of irrational application of TCM injections is relatively severe. Comprehensive intervention measures will be taken to gradually standardize the clinical application of TCM injections, improve the quality of medical services, and ensure the safety of medication for patients.
