对比交锁髓内钉内固定术与微创经皮钢板内固定术治疗胫骨骨折的临床有效性

     

摘要

目的 研究交锁髓内钉内固定术与微创经皮钢板内固定术治疗胫骨骨折的临床有效性.方法 将108例观察对象随机分为实验组和对照组,各54例.实验组给予交锁髓内钉内固定术治疗,对照组给予微创经皮钢板内固定术治疗,比较两种术式的疗效差异.结果 采用交锁髓内钉内固定术治疗的实验组患者中,痊愈21例(38.8%),显效19例(35.1%),有效11例(20.3%),无效3例(5.5%),总有效率为94.5%;采用微创经皮钢板内固定术治疗的对照组患者中,痊愈7例(12.9%),显效11例(20.3%),有效19例(35.1%%),无效17例(31.4%),总有效率为68.6%.治疗组治疗前1天、后1天、5天、3个月和6个月的ODI评分分别为(38.9±3.5)分、(15.6±1.5)分、(16.2±2.3)分、(15.6±3.5)分和(16.2±2.5)分;治疗组分别为(41.5±2.5)分、(16.2±3.2)分、(18.5±4.0)分、(36.5±3.6)分和(38.3±3.8)分.结论 交锁髓内钉内固定术治疗胫骨骨折相对于其他术式力学特性更优,疗效更佳,疼痛指数更低,值得借鉴推广.%Objective Research in interlocking intramedullary nail fixation with minimally invasive percutaneous plate fixation in the treatment of tibial fracture effectiveness.Methods 108 cases of observed objects randomly assigned into the experimental group and control group, 54 cases each. Experimental group give interlocking intramedullary nail fixation, the control group given minimally invasive percutaneous plate fixation treatment of tibial fractures, compare the curative effect of two kinds of operative methods.Results Using interlocking intramedullary nail fixation treatment of experimental group patients, 21 cases recovered (38.8%), 19 cases were markedly improved (35.1%), effective in 11 cases (20.3%), invalid in 3 patients (5.5%), the total effective rate was 94.5%; Using minimally invasive percutaneous plate fixation in treatment of the control group patients, cured 7 cases (12.9%), 11 cases were markedly improved (20.3%), 19 cases (35.1%), effective invalid 17 cases (31.4%), the total effective rate was 68.6%. Treatment group after 1 day, 1 day, 5 days ago, 3 months and 6 months of ODI score were (38.9±3.5)、(15.6±1.5)、(16.2±2.3)、(15.6±3.5) and (16.2±2.5); The treatment group were (41.5±2.5)、(16.2±3.2)、(18.5±4.0)、(36.5±3.6) and (38.3±3.8).Conclusion Locking intramedullary nail fixation compared with other surgical treatment of tibial fractures have better mechanical properties, curative effect more, pain index is lower and is worth popularizing for reference.
