门冬胰岛素治疗妊娠糖尿病的临床效果研究

摘要

目的：研究门冬胰岛素治疗妊娠糖尿病的临床效果。方法：选择来自江西省赣州市南康区第一人民医院2014年2月至2015年12月妊娠糖尿病患者100例。100例妊娠糖尿病患者随机分为常规人胰岛素组和门冬胰岛素组。所有患者均常规给予饮食控制，并增加运动量。在此基础上，常规人胰岛素组给予常规人胰岛素进行治疗，在三餐前半小时进行皮下注射；门冬胰岛素组给予门冬胰岛素进行治疗，在早、晚餐前5m i n进行皮下注射。比较两组患者治疗总有效率；血糖达标时间、胰岛素每天平均用量、治疗时间；用药前和用药后患者空腹血糖、餐后2h血糖、糖化血红蛋白(HbA1c)的差异；剖宫产率、羊水过多、胎膜早破、巨大儿等妊娠结局的差异。结果：门冬胰岛素组相较于常规人胰岛素组治疗总有效率更高，P0.05；用药后门冬胰岛素组相较于常规人胰岛素组空腹血糖、餐后2h血糖、HbA1c改善更显著，P0.05; fasting blood glucose, two hour postprandial blood glucose, glycosylated hemoglobin significantly improved in insulin aspart group compared with regular human insulin group,P<0.05. Cesarean section rate, polyhydramnios, premature rupture of membranes, the incidence of macrosomia was significantly lower than that of regular human insulin group Insulin aspart group,P<0.05.Conclusion: Insulin aspart can effectively reduce the maternal blood glucose level, promote blood glucose, reduce insulin dosage and treatment time, reduce the rate of cesarean section, improve outcomes, thus it is worthy of promotion.
