2型糖尿病患者血糖变异性与冠状动脉狭窄程度的相关性分析

摘要

Objective To investigate the correlation between blood glucose variability and coronary artery stenosis in type 2 diabetes. Methods 90 cases with type 2 diabetes admitted in our hospital from February 2013 to June 2014 were selected as the subjects and divided into the observation group(50 type 2 diabetic patients with coronary heart disease) and the control group(40 type 2 diabetic patients without coronary heart disease) in accordance with the result of coronary angiography. All the patients were given continuous glucose monitoring (CGM) for 72h after admission. And blood glucose fluctuation index was calculated in accordance with the value of CGM, Gensini integral was used to indicate the degree of coronary heart disease stenosis and SPSS18.0 statistical software was adopted to compare the general clinical indexes, CGM and Gensini integral.Results The difference in the course of disease, FPG, HDL-C, CRP, uric acid between the observation group and the control group was statistically significant(P<0.05); compared with the control group, the observation group had higher value of MBG, MAGE, MODD, LAGE and MPPGE with statistically significant difference(P<0.05); MBG, MAGE, MODD, LAGE and MPPGE were significantly correlated with Gensini integral. Conclusion Blood glucose variability is obviously correlated with the severity of coronary artery stenosis in type 2 diabetes, which can be used as an important indicator for evaluating the degree of coronary artery stenosis.%目的：探讨2型糖尿病患者血糖变异性与冠状动脉狭窄程度的相关性。方法选取2013年2月~2014年6月我院收治的90例2型糖尿病患者为研究对象,根据冠状动脉造影结果分为观察组(2型糖尿病合并冠心病,50例)和对照组(2型糖尿病不合并冠心病,40例)。所有患者入院后监测72h动态血糖(CGM),根据动态血糖值计算血糖波动指标,采用Gensini积分表示冠状动脉狭窄程度,采用SPSS18.0统计学软件比较两组患者一般临床指标、动态血糖指标及Gensini积分。结果观察组患者病程、FPG、HDL-C、CRP、尿酸和对照组比较有统计学意义(P<0.05)；观察组患者MBG、MAGE、MODD、LAGE及MPPGE各值均高于对照组,两组间各值比较有统计学意义(P<0.05)；MBG、MAGE、MODD、LAGE及MPPGE与Gensini积分呈明显相关性。结论2型糖尿病患者血糖变异性与冠状动脉狭窄程度有明显相关性,可作为预测冠状动脉狭窄程度重要指标。
