CT灌注成像对颈部淋巴结病变的诊断价值

摘要

目的 探讨CT灌注成像技术对颈部淋巴结病变的鉴别诊断价值.方法 对颈部恶性淋巴结组17例、良性淋巴结组13例行CT灌注成像扫描,盲法分析两组病例的时间密度曲线(TDC)和灌注参数的特点及差异.结果 良恶性病变组间的TDC形态不同,恶性淋巴结组的血容量(BV)和血流量(BF)值高于良性病变组,其差异均有统计学意义(P＜0.05).两组间表面通透性(PS)和平均通过时间(MTT)值的差异无统计学意义(P>0.05).恶性淋巴瘤与淋巴结转移瘤的BF、BV、MTT、PS值差异无统计学意义(P>0.05).结论 颈部良恶性淋巴结的CT灌注特点不同,CT灌注成像技术对颈部良恶性淋巴结病变的鉴别诊断有重要的应用价值,但对鉴别恶性淋巴瘤与淋巴结转移瘤价值有限.%Objective To explore the value of CT perfusion(CTP) in differential diagnosis of cervical lymph nodes. Methods CTP was performed in 30 objectives: 17 in malignant lymph node group, 13 in benign lymph node group. The time-density curve(TDC) and the data of perfusion parameters were recorded and analyzed statistically. Results There was a difference of the TDC features between the two groups. The average of blood volume(BV) and blood flow (BF) of the malignant groups were higher than those of benign groups(P< 0. 05), the average of mean transit time (MTT) and permeability surface ( PS) were lower than that of benign groups(P>0. 05). There were not differences of the BF, BV, MTT, and PS between the metastasic lymph nodes and lymphoma. Conclusion The features of the CTP are different in benign and malignant cervical lymph nodes, CTP is helpful for the differential diagnosis between the benign and malignant lymph nodes.However, CTP isn't helpful for the differential diagnosis between the metastasic lymph nodes and lymphoma.
