2型糖尿病合并脑梗死患者的临床分析

摘要

目的探讨合并与不合并2型糖尿病的脑梗死患者临床特点。方法选取合并与不合并2型糖尿病的脑梗死患者各80例，对两组患者的临床特点进行分析比较。结果2型糖尿病组患者高血压史、高血脂史、空腹血糖、甘油三酯、低密度脂蛋白、纤维蛋白原、糖化血红蛋白高于对照组患者，高密度脂蛋白低于对照组（P<0.05)。糖尿病组的颈动脉内膜中层厚度（IMT）较对照组增加（P<0.05)。糖尿病组中多灶性梗死、大面积梗死所占比例明显较对照组高(P<0.05)，而腔隙性脑梗死、混合性卒中及出血性梗死所占两组比例差异不显著，腔隙性脑梗死所占比例均明显高于其他梗死类型。结论2型糖尿病组患者的血脂代谢紊乱更为严重，颈动脉内膜中层厚度增加，应积极采取有效措施控制患者血压、血糖、血脂及纤维蛋白原。%Objective The purpose of this paper is to discuss the clinical features of cerebral infarction with type 2 diabetes mellitus patients or without. Methods Type 2 diabetic group(n=80 cases) and control group(n=80 cases) were chosen, the clinical characteristics of the two groups of patients were analyzed and compared .Results(1)Type 2 diabetic group patients with history of hypertension, history of high cholesterol, fasting glucose, triglycerides, low density lipoprotein, ifbrinogen was more than control patients;the high-density lipoprotein was less than control group(P<0.05).(2)The carotid intima-media thickness was significantly higher in the Type 2 diabetic group patients than control group(P<0.05).(3)The Multifocal infarction and large proportion of infarction in diabetes group was significantly higher than the control group(P<0.05),there was no signiifcant difference between the lacunar cerebral infarction, proportion of mixed stroke and hemorrhagic infarction in the two groups ,the proportion of lacunar cerebral infarction was signiifcantly higher than other type of infarction.Conclusion The patients with Type 2 diabetic sufferd in more lipid metabolism disorder, increased carotid intima-media thickness,and effect measures should be applied to actively control the blood pressure, the blood sugar, the blood lipids and the ifbrinogen .
