Objective To investigate the clinical efifcacy and safety of Colonoscopy noninvasive treat acute appendicitis. Methods 60 patients in our hospital were Choiced in July 2012-July 2014, and were randomly divided into experimental group and the control group (n=30) for different treatment methods, the experimental group treated by endoscopic retrograde, control group adopts the laparoscopic appendectomy, comparing the clinical curative effect and adverse reactions between two groups. Results The hospital time, anal exhaust time, the treatment expense obviously in experimental group are lower than the control group, the differences have statistics signiifcance (P<0.05);The experimental group abdominal pain relieves the number of days average (4.0±1.2)d, the control group abdominal pain relieves the number of days average (6.9±1.7)d, the comparison differences have statistics signiifcance between two groups(P<0.05);The experimental group does not have an example to have the complication, after the control group technique has the margin to infect 2 examples, 1 example has the adhesion intestinal obstruction. Conclusion Colonoscopy under noninvasive treat acute appendicitis curative effect is good, quick recovery, low cost, no obvious adverse reactions, it worth for clinical popularization and application.%目的：探讨结肠镜下无创治疗急性阑尾炎的临床疗效及安全性。方法选择60例急性阑尾炎患者，按治疗方法的不同随机均分为实验组和对照组（n=30）。实验组采用内镜逆行治疗，对照组采用腹腔镜阑尾切除，比较2组患者的临床疗效及不良反应。结果实验组住院时间、术后肛门排气时间、治疗费用均明显低于对照组，2组比较差异具有统计学意义（P<0.05）；实验组腹痛解除天数平均（1.5±0.5）d，对照组腹痛解除天数平均（4.1±0.7）d，2组比较差异具有统计学意义（P<0.05）；实验组无1例发生并发症，对照组术后发生切口感染2例，1例发生粘连性肠梗阻。结论结肠镜下无创治疗急性阑尾炎疗效好、恢复快、费用低，无明显不良反应，值得临床推广应用。
