武警某部医院工作人员疲劳状况调查分析

         

摘要

目的 调查军队医院工作人员疲劳状况及相关因素,旨在为军队医院医护人员健康维护、心理干预和医院建设提供理论依据.方法 采用一般情况调查表、疲劳评定量表(fatigue assessment instrument,FAI)、疲劳量表-14(fatigue scale,FS-14)、简易应对方式问卷(simplified coping style questionnaire,SCSQ)和社会支持评定量表(social support rating scale,SSRS)对武警某部医院200名工作人员进行调查分析.结果(1)该武警部队医院军人的GFSS、SSFS、PF、MF、FS显著低于合同制人员(均P<0.05);医护人员的GFSS、SSFS、FCS、PF、MF、FS显著高于工勤人员(均P<0.01);女性医护人员GFSS、SSFS、FCS、RRS、PF、FS显著高于男性医护人员(均P<0.01);护士的SSFS和RRS显著高于医生(均P<0.01),护士的MF显著小于医生(P<0.01).(2)医院工作人员的GFSS与消极应对呈显著负相关(r=-0.159,P<0.05);SSFS、FCS、PF、MF、FS与主观支持呈显著负相关(r=-0.150,-0.246,-0.180,-0.183,-0.217,P<0.05);FCS与支持总分呈显著负相关(r=-0.150,P<0.05).(3)影响疲劳感的因素有性别、年龄、工作年限、社会支持总分和婚姻状况(t=2.275,4.509,-4.203,1.970,8.507,-8.266,-2.151,P<0.05).决定系数R2为0.345和0.431,可解释总变异量的34.5%和43.1%.结论 医院合同制人员、女性医护人员、护士的疲劳严重程度高,应对方式和社会支持差.女性、年龄大、工作年限短、已婚或离异、社会支持差者容易出现疲劳,需加强心理干预,改善认知,增强社会支持.%Objective To investigate the fatigue status and its related factors on staff of an armed police hospital,so as to provide theoretical basis for medical workers' health maintenance, psychological intervention and hospital development.Methods A total of 200 workers of an armed police hospital were investigated by general status,fatigue assessment instrument,fatigue scale-14,simplified coping style questionnaire and social support rating scale.Results (1)The scores of GFSS,SSFS,PF,MF and FS of military of the armed police hospital were lower than those of contract staff(t=-4.501--2.544;P<0.05). The scores of GFSS,SSFS,FCS,PF,MF and FS of medical staff were higher than those of general workers(P<0.01). The scores of GFSS,SSFS,FCS,RRS,PF and FS of female medical staff were higher than those of male medical staff (P<0.01). The scores of SSFS and RRS of nurses were higher than those of doctors(P<0.01). The score of MF of nurses was lower than it of doctors (P<0.01).(2)The GFSS of hospital staff was negatively correlated with negative coping (r=-0.159,P<0.05).The SSFS,FCS,PF,MF and FS were negatively correlated with subjective support (r=-0.246-0.150,P<0.05).The FCS was negatively correlated with total score of support (r=-0.150;P<0.05).(3)The factors of fatigue influences were gender,age,working years,total score of social support and marital status (t=-8.266,-8.507,P<0.05). R2 were 0.345 and 0.431,they could explain total ariance of 34.5 percent and 43.1 percent.Conclusion The fatigue severity ofcontract staff,female medical staff and nurses were higher.Their coping styles and social support were poor.The staff of female,elders,shorter working life,married or divorced and lower social support were easy to have fatigue.It should strengthen psychological intervention,to improve cognitive and reinforce social support.
