Objective To explore the significance of detection of MCV and MCH in the screening of thalassemia. Methods A total of 3 000 cases of thalassemia screening in our hospital from September 2014 to September 2015 were selected. All cases received gene test of thalassemia, and detection of MCV and MCH. With gene test results as the reference standard, the accuracy, specificity, sensitivity, positive predictive value and negative predictive value of MCV, MCH and their combination were calculated. Results The sensitivity and specificity were 91.2%and 76.1%of MCV detection respectively, and 91.2%and 70.7%of MCH detection respectively. The sensitivity and specificity of parallel detection of MCV and MCH were 96.0%and 65.4%respectively, and the sensitivity and specificity of series detection of MCV and MCH were 95.9%and 85.0%respectively. Conclusions The combined detection of MCV and MCH has high sensitivity in the screening of thalassemia. Combination of gene test and MCV, MVH detection has important diagnostic value.%目的:研究地中海贫血筛查中检测MCV和MCH指标的临床意义。方法临床纳入3000例我院2014年9月至2015年9月期间接受筛查的受试者,均给予地中海贫血的基因检测。以地中海贫血基因的检测结果作为参考标准,计算并观察MCV和MCH指标单项或联合检测的准确性、特异度、灵敏度、阳性预测值和阴性预测值。结果 MCV单项检测的灵敏度和特异度分别是91.2%、76.1%, MCH单项检测的灵敏度和特异度分别是91.2%、70.7%, MCV+ MCH平行和系列检测地中海贫血的灵敏度和特异度分别是96.0%和65.4%、95.9%和85.0%。结论 MCV、 MCH联合检测在地中海贫血筛查中具有较高的灵敏度,结合临床基因诊断具有重要的临床意义,值得推广。
