目的 探讨社区糖尿病患者血脂谱情况, 分析糖尿病患者影响血糖控制的相关因素.方法 通过社区2017年1月1日—12月31日居民体检, 将糖尿病患者分为糖尿病管理患者400人、糖尿病非管理患者116人和糖尿病合并高血压管理患者432人三类, 比较三类人群血糖、血压、体质指数和血生化指标, 通过非条件logistic逐步回归模型分析影响血糖达标率的因素.结果 糖尿病规范化管理患者血压、体质指数、空腹血糖、高密度脂蛋白、甘油三酯和尿酸指标与非管理糖尿病患者有差异 (P<0.05);非条件logistic逐步回归模型分析显示:高血压 (OR=2.313, 95%CI:1.715~3.118) 、年龄 (OR=0.958, 95%CI:0.940~0.976) 、甘油三酯 (OR=1.246, 95%CI:1.074~1.446) 、高密度脂蛋白 (OR=0.615, 95%CI:0.395~0.957) 、尿酸 (OR=0.997, 95%CI:0.995~0.998) 和收缩压 (OR=1.009, 95%CI:1.001~1.017) 是糖尿病管理患者血糖控制的影响因素.结论 社区糖尿病规范化管理能够改善糖尿病患者血糖、血压和血脂等指标, 针对目前糖尿病血糖控制影响因素, 还需要不断优化管理规范.%OBJECTIVE To investigate the blood lipid profile of diabetic patients in community, and to analyze the related factors affecting blood glucose control in diabetic patients. METHODS According to the residents' physical examination from January 1 2017 to December31 st in 2017, the diabetic patients were divided into 400 diabetes management patients, 116 diabetic non-management patients and432 patients with diabetes and hypertension management. Then it compared the blood sugar, blood pressure, body mass index and blood biochemical index of the three groups. The factors affecting the blood sugar standard rate were analyzed by the unconditional logistic stepwise regression model. RESULTS Patients with standardized diabetes management had differences in blood pressure, body mass index, fasting blood glucose, high-density lipoprotein, triglyceride and uric acid index (P<0.05). Non conditional logistic regression analysis showed that the hypertension (OR=2.313, 95% CI:1.715~3.118), age (OR=0.958, 95% CI:0.940~0.976), triglyceride (OR=1.246, 95% CI:1.074~1.446), high density lipoprotein (OR=0.615, 95% CI:0.395~0.957), uric acid (OR=0.997, 95% CI:0.995~0.998) and systolic blood pressure (OR=1.009, 95% CI:1.001~1.017) were influencing factors of blood glucose in patients the management of diabetes control. CONCLUSION Standardized management of community diabetes could improve blood sugar, blood pressure, blood lipid and other indicators in diabetic patients. In view of the influencing factors of diabetes control in diabetes, it needed to optimize the management standards continuously.
