自我管理教育在心脏移植患者出院后康复过程中的应用

         

摘要

目的:评价自我管理教育在心脏移植患者康复过程中的应用效果。方法:对30例心脏移植术后患者实施自我管理教育;采用慢性病管理自我效能量表和简明健康状况量表对患者在出院时,出院后3个月、6个月进行测评。结果:患者的自我效能得分和生活质量得分在出院后3个月、6个月时明显提高,出院后6个月的生活质量得分接近正常人群。结论:自我管理教育有助于提高心脏移植术后患者的自我效能以及生活质量。%Objective: To evaluate the effect of self-management education among patients with heart transplantation during post-hospital rehabilitation process. Methods: Totally 30 patients were received self-management education and investigated using the Chronic Disease Management Self-efifcacy Scale and Short Form 36 Health Survey Questionnaires (SF-36) before discharge, 3 months and 6months after discharge. Results:The scores of the two scales were signiifcantly improved after 3 months and 6 months, and the score of SF-36 after 6 months was close to normal people. Conclusion: The patients after heart transplantation need self-management education. Self-management could help patients improve their self-efifcacy and quality of life.
