晚期妊娠脐血流异常胎儿心脏功能的超声评估

摘要

目的 研究彩色多普勒超声对晚期妊娠脐血流异常胎儿心脏功能的评估价值.方法 选取2014年1月至2015年12月在西北妇女儿童医院产前检查的晚期妊娠脐血流异常胎儿88例作为观察组,选取同期产前检查无妊娠合并症和并发症100例胎儿作为对照组,采用彩色多普勒超声测量Tei指数和射血分数(EF).脐血流异常胎儿出生后随访心脏功能.结果 ① 观察组胎儿左室Tei指数(观察组:0.52±0.06;对照组:0.38±0.11)、右室Tei指数(观察组:0.54±0.75;对照组:0.37±0.12)均高于对照组(t值分别为2.22、2.33,均P<0.05);② 观察组胎儿EF与对照组比较差异无统计学意义(P>0.05).结论 晚期妊娠脐血流异常胎儿存在心脏功能损害;Tei指数评价晚期妊娠脐血流异常胎儿心脏功能受损早于EF.%Objective To evaluate the significance of ultrasonic examination on cardiac function of fetus with abnormal umbilical blood flow in the third trimester .Methods During the third trimester ,88 fetuses with abnormal umbilical blood flow were chosen as study group and 100 normal fetuses without complications as control group from January 2014 to December 2015 in Northwest Women's and Children's Hospital .Tei index and ejection fraction (EF) were measured by color Doppler ultrasound .The cardiac function of fetus in study group was followed up after born .Results Tei indexes of the right (0 .52 ± 0 .06 vs 0 .38 ± 0 .11) and left ventricle (0 .54 ± 0 .75 vs 0 .37 ± 0 .12) of the study group were higher than those of the control group (t value was 2 .22 and 2 .33 , respectively ,both P < 0 .05) .There was no significant difference in EF between two groups (P > 0 .05) .Conclusion The cardiac function of fetus with abnormal umbilical blood flow in the third trimester is damaged .Abnormal fetus' damages in cardiac function in the third trimester can be evaluated earlier by Tei index than EF .
