彩色多普勒超声对左肾静脉受压综合征的诊断探讨

         

摘要

Objective To explore the value of the color Doppler ultrasound in left renal vein compression syndrome (nut-cracker syndrome, referred to as NCS). Methods 100 healthy individuals (control group) and 74 cases of NCS patients (case group) were measured and performed in the supine position, standing backward extension and the kneeling position. Inter-segment diameter (a) of left renal vein (LRV) which between the abdominal aorta (AO)and superior mesenteric artery (SMA) and LRV diameter near kidney side (b) were measured, b/a value was calculated. The maximum velocity (Va, Vb) of a, b segment, narrowest peak venous pressure (ΔP) and angle between the formation of AO and SMA in supine position were measured, two group of data were compared. Results With supine and standing position, b/a value of case group was larger than that of the control group; b/a ≥3, 59 cases (79.73%) in case group, while 4 cases in the control group; ΔP of case group showed significantly higher compared with that of the control group; Difference between the above-mentioned parameters in two groups were significant (P<0. 01). The angle between AO and SMA in case group were less than 30°, 87.84% of cases ≤20°. Conclusions The combination with the supine position and standing backward extension ultrasound examination can provide an objective evidence and significant value for the diagnosis of NCS.%目的 探讨彩色多普勒超声诊断左肾静脉受压综合症(nut-cracker syndrome,简称NCS)的价值.方法 对100例健康体检者(对照组)与74例NCS患者(病例组)仰卧位、站立背伸位及跪位时,测量左肾静脉(LRV)位于腹主动脉(AO)和肠系膜上动脉(SMA)问段内径(a)、LRV近肾端内径(b),计算b/a值.测量a、b段最大流速(Va、Vb);计算最窄处峰值静脉压(△P).测量仰卧位时AO和SMA形成的夹角,将两组所得数据进行对比分析.结果 仰卧位和站立位时,b/a值病例组较对照组大;b/a≥3者病例组有59例(79.73%),而对照组有4例(4.00%);站立背伸位检查,b/a≥4者病例组有72例(97.30%),而对照组0例;AP示病例组比对照组明显增高;以上参数两组间比较差异均有显著性(P<0.01).病例组AO和SMA夹角均小于30°,87.84%病例≤20°.结论 通过仰卧位及站立背伸位超声联合检查,可为诊断NCS提供客观依据,对NCS的诊断具有重要价值.
