宫腔粘连宫腔形态的三维超声观察

摘要

Objective To retrospective analysis endometrial cavity features by transvaginal three-dimensional ultrasonography in patients with intrauterine adhesions and improve the understanding of ultrasound images of intrauterine adhesions.Methods 51 patients with intrauterine adhesions confirmed by hysteroscopy were examined by transvaginal three-dimensional ultrasonography,endometrial cavities shape and structure were observed in the coronal plane.Results In three-dimensional coronal plane,the normal inverted triangle performance was disappear in intrauterine adhesions,it displayed as endometrial defects with different degree,fundus and two sides of the endometrial cavities lose smooth edges.Angularity of the two sides of the endometrial cavities was the typical features,among them,10 cases in grade Ⅱ (10/16,62.5％),6 cases in grades Ⅲ (6/8,75％),10 cases in grade Ⅳ (10/10,100％),17 cases in grade Ⅴ (17/17,100％),endometrial cavities were showed as T-shaped in grade Ⅳ and grade Ⅴ.Conclusions In three-dimensional coronal plane,angularity of the two sides of the endometrial cavities is specific sign in intrauterine adhesions,T-shaped endometrial cavities is the sign of severe intrauterine adhesions.%目的 回顾性分析官腔粘连患者官腔形态的经阴道三维超声图像特点,提高对官腔粘连超声图像的认识.方法 经官腔镜确诊的官腔粘连患者51例,所有患者留取二维声像图后行经阴道三维超声检查,在冠状面观察官腔形态及结构.结果 官腔粘连患者三维冠状切面失去正常倒三角形的表现,显示内膜不同程度缺损,宫底及两侧壁官腔失去平滑边缘;官腔形态内聚成角为官腔粘连的典型声像图特征,其中Ⅱ级10例(10/16,62.5％),Ⅲ级6例(6/8,75％),Ⅳ级10例(10/10,100％),Ⅴ级17例(17/17,100％),Ⅳ级和Ⅴ级官腔呈T型.结论 三维冠状切面官腔两侧边内聚成角是官腔粘连的特异度征象,官腔呈T型为重度官腔粘连的征象.
