超声造影在胰周淋巴结结核诊断中的应用价值

摘要

Objective To evaluate the characteristics of Peripancreatic Lymphoid tuberculosis with contrast-enhanced ultrasound (CEUS).To explore the value of Contrast-enhanced Ultrasound in diagnosing Peripancreatic Lymphoid Tuberculosis.Methods Twenty-three patients of Peripancreatic Lymphoid tuberculosis proved by biopsy or surgery were studied with Contrast enhanced Ultrasound and summarized the characteristics.To make the diagnosis according to the characteristics and compare with CT diagnosis.Results The accuracy of Contrast enhanced Ultrasound diagnosis in Peripancreatic Lymphoid Tuberculosis was ninety one point three percent.The accuracy of CT diagnosis in Peripancreatic Lymphoid Tuberculosis was eighty six point nine percent.There was no significant difference between the two(P＞0.05).The results of CEUS were divided into three types:annular enhancement,uneven enhancement and separated enhancement.Fourteen cases of the first type displayed annular enhancement of the mass and inside region with no enhancement.Six cases of the second type showed uneven enhancement homogeneous which had distributed and reflected the different intensity in the all area.Three cases of the third type displayed margin enhancement and inside region with slight separated enhancement.There was no significance in the arrival time and time-to-peak with three types(P＞0.05).Conclusions Contrast-enhanced Ultrasound in diagnosing Peripancreatic Lymphoid Tuberculosis has a certain application value and can provide the reliable basis for clinical diagnosis.%目的 评价胰腺周围淋巴结结核的超声造影声像图特征,探讨超声造影在胰腺周围淋巴结结核诊断中的应用价值.方法 回顾性分析23例经穿刺活检或手术病理证实为胰腺周围淋巴结结核的声造影声像图特征,根据声像图特征做出超声诊断,并与CT诊断结果比较.结果 超声造影诊断胰腺周围淋巴结结核的准确率为91.3％(21例),CT诊断胰腺周围淋巴结结核的准确率为86.9％ (20例),二者之间差异无统计学意义(P＞0.05);胰腺周围淋巴结结核超声造影主要表现为3型:环状增强型14例,病灶边缘快速强化,内部无强化;不均匀增强型6例,病灶强化不均匀,部分区域可见强化;分隔样增强型3例,表现为病灶周边快速强化,内部条带状增强,呈分隔样.三种类型的开始强化时间和达峰时间之间比较无明显差异无统计学意义(P＞0.05).结论 超声造影对胰腺周围淋巴结结核的诊断具有一定的应用价值,为临床诊断和治疗提供可靠依据.
