高血压脑出血手术时间及方式对手术效果的影响

摘要

目的：观察高血压脑出血手术时间及方式对手术治疗效果的影响。方法120例高血压脑出血患者根据手术时间、手术方式不同分为观察组和对照组，观察组接受超早期微创血肿清除术，对照组接受延期开颅血肿清除术，比较2组手术情况、炎症应激指标以及神经功能恢复情况。结果观察组手术时间、术中出血量、术后意识恢复时间、术后引流量、住院总时间、ACTH、Cor、NE、CRP、TNF‐α、CSS评分、NIHSS评分均明显低于对照组（P＜0.05）。结论超早期微创血肿清除术有助于减小手术创伤，促进术后恢复，缓解炎症应激反应，改善神经功能，是高血压脑出血理想的手术方式。%Objective To study the influence of operation time and type on patients with hypertensive cerebral hemor‐rhage .Methods 120 cases with hypertensive cerebral hemorrhage from April 2012 to April 2014 in our hospital were enrolled and divided into observation group and control group according to different duration and type of operation .Patients in observa‐tion group received micro‐traumatic hematomas cleaning operation at ultra‐early period ,the control group patients received de‐layed hematoma evacuation .Then operation situation ,inflammation‐stress index and the recovery of neurological function were compared .Results In observation group ,the peration time ,intra‐operative bleeding volume ,postoperative cons‐ousness re‐covery time ,drainage volume ,hospitalization time ,and the serum concentration of ACTH ,Cor ,NE ,CRP ,TNF‐α,CSS score ,and NIHSS scores were significantly lower than those in control group .Conclusion The early minimally invasive sur‐gery is helpful to reduce operation trauma ,promote postoperative recovery ,alleviate inflammatory and stress reaction ,improve nerve function .It's an ideal operation method in treating hypertensive cerebral hemorrhage .
