新诊断2型糖尿病患者血清25羟维生素D3与HbA1c及胰岛β细胞功能的关系

摘要

目的 探讨新诊断2型糖尿病患者血清25羟维生素D3[25(OH)D3]的水平与糖化血红蛋白(HbA1c)和胰岛β细胞功能的关系.方法 选择2015年11月至2016年3月新诊断2型糖尿病患者107例,于晨起7∶00抽取肘静脉血,检测一般生化指标、血清25(OH)D3、HbA1c,并行OGTT+C肽释放试验.HbA1c≥9%为A组,HbA1c<9%为B组; 25(OH)D3<18 ng/ml为a组,≥18 ng/ml为b组.分析其一般资料及生化指标、25(OH)D3水平及HbA1c、胰岛β细胞功能的关系.结果 维生素D缺乏者占72%(77/107),不足者占27%(29/107),正常者占1%(1/107).不同性别糖尿病患者维生素D水平和缺乏程度比较差异未见统计学意义(t=0.394、0.91,P>0.05).A组25(OH)D3水平较B组明显下降(t=5.71,P<0.05),HbA1c与25(OH)D3水平呈负相关.a、b两组胰岛β细胞功能指数、胰岛素抵抗指数、胰岛素敏感指数比较差异未见统计学意义(P>0.05).结论 初诊2型糖尿病患者普遍存在维生素D的缺乏,血清25(OH)D3水平与HbA1c呈负相关,与胰岛β细胞功能无相关性.%Objective To investigate the relationship between islet beta cell function, glycosylated hemoglobin (HbA1c) and the serum 25 hydroxy vitamin D3 [25(OH)D3] in newly diagnosed type 2 diabeic patients.Methods A total of 107 patients with newly diagnosed type 2 diabetes were selected from November 2015 to March 2016, Biochemistry, 25(OH)D3, HbA1c, OGTT and C-peptide release test were carried out on the patients include at 7∶00. The patients were divided into two groups, HbA1c≥9% was group A,HbA1c<9% was group B; 25(OH)D3<18 ng/mL was group a, ≥18 ng/ml was group b. The data of general information(gender, age, BMI, biochemistry) and glucose metabolism(HbA1c, HOMA-B, HOMA-IR, HOMA-S) and 25(OH)D3 were analyzed.Results Vitamin D deficiency was 72%(77/107), insufficient was 27%(29/107), and sufficient was 1%(1/107). The difference was not significant between the females and males in the level of 25(OH)D3(t=0.394,P>0.05). Serum 25(OH)D3 level of group A was lower than that of group B(t=5.71,P<0.05, glycosylated hemoglobin was negatively correlated with 25(OH)D3 level. After grouping the patients into groups according to the level of 25 (OH) D3, the differences were not significant among the HOMA-B, HOMA-IR, HOMA-S of the two groups.Conclusions It is common in newly diagnosed type 2 diabetes patients with a lack of vitamin D, HbA1c is negatively correlated with serum 25(OH)D3 level. The level of serum 25 (OH) D3 is not correlated with the function of pancreatic islet.
