儿童脑内小脓肿的诊断和治疗

摘要

Small intracerebral abscess less than 1.5 cm in diameter,is a disease of its special form of brain abscess.Clinically,it is characterized by sudden onset of focal epilepsy but negative findings in CT.The present series consists of 12 cases,2 treated conservatively and 10 operatively.The key point of successful surgery depends on the accurate locating of the abscess.Our self-making multi-holed measuring board has proved to be helpful.The postoperative sequelae,such as epilepsy and paralysis are discussed,and suggestions for improving the surgical result are put forward.%本文报告12例儿童脑内小脓肿(直径在1.5cm以下),不同于一般脑脓肿.指出其临床特点为:①病前无明显的感染史或外伤史.②临床表现多以癫痫为首发症状,且多为局灶性癫痫发作.③CT扫描具有特征性改变,是诊断本病的可靠方法.本组12例中2例采用非手术治疗,10例手术治疗,无手术死亡率.作者认为术中准确定位是手术能否成功和避免手术并发症的关键.作者采用自制多孔测量板术前依据病变在CT扫描层的前后、左右的距离进行定位,10例手术均获得成功,并对手术后癫痫发作加以讨论.
