Objective Comparative analysis advantages and disadvantages for chronic hypertrophic rhinitis of two surgical methods. Mothods 138 Chronic hypertrophic rhinitis patients who in our hospital ENT head and neck department Since August 2008 to August 2012 were randomly divided intotwo groups:group A were 72 cases of inferior turbinate sticky subperiosteal resection,and group B were 66 cases of plasma ablation. Follow-up more than 12 months,endoscope to review after 3 months,observing turbinate morphological changes of the two groups,using VAS scale to two groups for subjective analysis of pre-operation,3 months after surgery and 12 months after surgery,and through rhinomanoetry for objective analysis of curative effect between the two groups. Result Two surgical methods can both improve symptoms, after 3 months the inferior turbinate volume shrinking,no matter VAS scale or morphologicalhave shown that short-term effectswerenot significantly different between the two groups (P>0.05), But the group A patients had better effect than the plasma ablation group for long-term (P<0.05). Conclusion Inferior turbinate resection of sticky subperiosteal and plasma ablation are all effective in the treatment of chronic hypertrophic rhinitis.The former can better keep the turbinate mucosa, glands,hair and avoid scar wound healing, which preservating of nasal function maximum, at the same time relieving nasal obstruction, and long-term results are satisfactory. It is worthy of clinical popularization and can be used as the preferred operation of inferior turbinate.%目的 比较分析针对慢性肥厚性鼻炎两种手术方法在临床应用中的优缺点.方法 将我科自2010年8月~2012年8月收治的138例慢性肥厚性鼻炎患者随机分成2组, 其中A组72例行下鼻甲骨黏膜下部分切除术,B组66例行下鼻甲等离子射频消融术,随访12个月以上,术后3月进行鼻内镜复查,观察两组患者下鼻甲形态改变,分别于术前、术后3个月、12个月运用VAS量表对两组术后临床症状改善进行主观分析,并行鼻阻力测定对两组间疗效进行客观分析.结果 两组手术方式均能够有效改善症状,术后3月下鼻甲体积缩小;无论是VAS量表还是鼻阻力测定,结果均提示两组间近期效果无明显差异(P>0.05),但下鼻甲骨黏膜下切除的患者远期效果要优于等离子消融(P<0.05);结论 下鼻甲骨黏膜下切除术和等离子射频消融术是慢性肥厚性鼻炎有效的治疗方法,前者能更好的保留鼻甲黏膜、腺体、纤毛,避免创面的瘢痕愈合,最大程度上保留鼻功能同时解除鼻阻,远期效果理想,值得临床逐步开展推广,可作为下鼻甲手术的首选.
