目的 探讨气管异物的类型和手术难度相干性,总结出一定的规律,对术前的评估和手术器械的选择起到指导作用.方法 通过详细询问病史,了解气管异物吸入的种类,时间的长短;通过阅读CT片,了解气管异物的大小,位置;通过详细的体格检查,了解呼吸音的变化,呼吸困难的程度;对患儿术前进行详细的评估和选择合适的手术器械.结果 127例患儿均一次手术成功取出异物,无气胸、气管内出血、异物残留、双侧支气管异物漏诊、气管痉挛、窒息等并发症出现,无死亡病例.结论 通过对气管异物的类型和手术难度的评估分析,使术者术前对手术的难度有充分的了解和心里把握,使手术医生有充分的心理准备并选择合适的手术器械从容进行手术操作,可提高手术的精确性和安全性,避免因紧张或带来的意外损伤和手术并发症的发生.%Objective To explore the types of tracheal foreign bodies and the difficulty of operation and summarize the certain rules which can guide the preoperative evaluation and the selection of surgical instruments. Methods The types of foreign bodies and the length of time were investigated by means of detailed medical history. The size and location of the foreign bodies were obtained through CT. The change of breath sounds and the degree of difficulty in breathing were understood by careful examination. Detailed evaluation and selection of appropriate surgical instruments were performed before operation for these children. Results One hundred and twenty-seven cases of children were successfully removed foreign bodies with only one operation. No?patients?died and no pneumothorax, endotracheal hemorrhage, foreign body residues, bilateral bronchial foreign bodies missed diagnosis, tracheal spasm, asphyxia and other complications occurred. Conclusion With the evaluation and analysis of the tracheal foreign body type, the surgeon can determine the difficulty of the case before operation, which is helpful for the selection of appropriate surgical instruments. These measures can also improve the accuracy and security of the operation and?reduce?the chance?of surgical complications due to tension or blind operation.
