非跨关节外固定架治疗胫骨平台骨折临床应用研究

摘要

目的 探讨非跨关节外固定架治疗胫骨平台骨折临床应用研究. 方法 选取2012年1月至2016年1月期间我院确诊治疗的胫骨平台骨折患者46例,依据随机数表法随机分为非跨关节组和常规组,每组23例,常规组患给予常规外固定架治疗,非跨关节组患者给予非跨关节外固定架治疗,采用美国特种外科医院膝关节评分(hospital for special surgery knee score,HSS)评估治疗疗效,统计分析所有患者治疗疗效、负重锻炼时间、骨折愈合时间和并发症发生情况. 结果 非跨关节组患者负重锻炼、骨折愈合时间明显低于常规组,差异有统计学意义(P＜0.05),但两组并发症发生率差异无统计学意义(P＞0.05);非跨关节组患者治疗优良率明显高于常规组,差异有统计学意义(P＜0.05). 结论 非跨关节外固定架治疗可有效缩短胫骨平台骨折患者负重锻炼时间和骨折愈合时间,有利于患者膝关节功能的恢复,且具有良好的安全性,值得临床作进一步推广.%Objective To discuss the clinical application of the non-cross joint external fixators in tibial plateau fractures.Methods 46 patients with tibial plateau fractures were selected from January 2012 to January 2016 in our hospitals.According to the random table,all patients were divided into the non-cross joint group and the routine group,23 cases in each.The routine group was fixed with conventional external fixators,and the non-cross joint group was fixed with the non-cross joint external fixators.Hospital for special surgery knee score (HSS) was used to evaluate the curative effect,and weight-bearing exercise time,fracture healing time and complications were recorded as well.Results Weight-bearing exercise time and fracture healing time of non-cross joint group patients was significantly shorter than that of the routine group,and the differences were statistically significant (P ＜0.05).But there was no statistically significant difference in the complication rates of the two groups (P ＞ 0.05).The good rate of non-cross joint group patients was significantly higher than that of the routine group,and the difference was statistically significant (P ＜ 0.05).Conclusions The non-cross joint external fixator can effectively shorten healing time of tibial plateau fractures and weight-bearing exercise time.It is beneficial to the recovery of the knee joint function with good security.It's worth further clinical promotion.
