目的:观察机械辅助排痰系统对重症加强护理病房(ICU)机械通气患者排痰效果的影响。方法80例 ICU 行机械通气的患者,随机分为观察组及对照组,各40例。两组患者均给予基本治疗及护理,在此基础上,对照组患者采用传统的手工叩背排痰法,观察组应用机械辅助排痰系统排痰,观察记录两组患者治疗5 d 后的血氧饱和度(SpO2)、动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)及两组患者机械通气时间。结果治疗5 d 后,观察组患者的 SpO2、PaO2高于对照组, PaCO2低于对照组,差异均有统计学意义(t=3.668、5.886、9.844, P<0.05)。观察组机械通气时间为(90.35±8.20)h,短于对照组的(121.36±7.90)h,差异有统计学意义(P<0.05)。结论机械辅助排痰系统能够明显改善患者的呼吸通气,减少患者机械通气的时间,值得临床推广。%Objective To observe influence of mechanical-assisted sputum excretion system on effect of sputum excretion in intensive care unit (ICU) patient with mechanical ventilation. Methods A total of 80 ICU patients with machinery ventilates were randomly divided into observation group and control group, with 40 cases in each group. Both groups received basic treatment and nursing, and the control group also received conventional manual knocking back method for sputum excretion, while the observation group received mechanical-assisted sputum excretion system. Observation were made on saturation of blood oxygen (SpO2), arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2) and duration of mechanical ventilation in 5 d ofter treatment between two groups. Results After 5 d of treatment, the observation group had higher SpO2 and PaO2 than the control group, and lower PaCO2 than the control group, and their differences had statistical significance (t=3.668, 5.886, 9.844, P<0.05). The observation group had duration of mechanical ventilation as (90.35±8.20) h, which was shorter than (121.36±7.90) h in the control group, and the difference had statistical significance (P<0.05). Conclusion Mechanical-assisted sputum excretion system can clearly improve respiratory ventilation of patients and reduce duration of mechanical ventilation. So this method is worthy of clinical promotion.
