肝圆韧带在腹腔镜上消化道穿孔手术中的应用

摘要

目的：探讨腹腔镜下使用上腹部带血管蒂的肝圆韧带修补上消化道穿孔的应用。方法回顾性分析2014年2月～2015年8月腹腔镜下使用带蒂的肝圆韧带行上消化道穿孔修补术51例的临床资料，均术中发现无法使用大网膜修补。结果所有病例均顺利完成腹腔镜手术，无胃肠漏和腹腔脓肿，均顺利出院。结论腹腔镜行上消化道穿孔修补术安全，有效。肝圆韧带作为上腹部较为游离的韧带在腹腔镜下行上消化道穿孔修补术中是很好的生物补片材料。%Objective To explore the application of the ligamentum teres hepatis in laparoscopic treatment for upper gastrointestinal perforation. Methods Clinical data of 5 1 patients with upper gastrointestinal perforation who were found unable to repair with the greater omentum during laparoscopic treatment from February 2014 to August 2015 were retrospectively analyzed.All the cases were repaired with the pedicled ligamentum teres hepatis. Results Laparoscopic surgery was successfully completed in all the cases without gastrointestinal leakage and abdominal abscess.The patients were discharged from the hospital smoothly. Conclusions Laparoscopic repair of upper gastrointestinal perforation is safe and effective.The ligamentum teres hepatis can be used in laparoscopic treatment of upper gastrointestinal perforation.
