尼龙丝检查对糖尿病周围神经病变早期诊断价值的探讨

摘要

目的 探讨10g尼龙丝检查对糖尿病周围神经病变(DPN)的早期诊断价值.方法 对133例2型糖尿病患者进行神经传导速度(NCV)检查及10g尼龙丝检查,以NCV检查作为"金标准",计算两者的一致性Kappa值.结果 10g尼龙丝检查异常率为22.56%,与NCV检查诊断DPN的一致性Kappa值为0.155.结论 10g尼龙丝检查不适合DPN的早期诊断,更适合晚期DPN和足溃疡的筛查.%Objective To investigate the early diagnostic value of 10g monofilament examination to diabetic peripheral neuropathy (DPN). Methods Examination of nerve conduction velocity (NCV) and 10g monofilament test were carried out in 133 type 2 diabetic patients. The outcomes of NCV were taken as the golden standard, and kappa value was analyzed for 10g monofilament test. Results The frequency of abnormality in 10g monofilament examination was 22. 56％, and the kappa value was 0. 155. Conclusions 10g monofilament examination may not be fit for screening DPN in early stage, but for advanced stage or foot ulcer.
