宫腔镜手术中膨宫压力对子宫内膜细胞播散的影响

摘要

目的:探讨膨宫压力在宫腔镜手术中对子宫内膜细胞腹腔播散的影响,从而寻找安全膨宫压力下宫腔镜检查技术.方法:收集我院140例行宫腹腔镜联合手术的患者,随机分为A、B两组,每组70例,A组膨宫压力为(80～120)mmHg,B组膨宫压力为(120～160)mmHg,分别在A、B两组宫腔镜手术前后在腹腔镜直视下抽取盆腹腔灌洗液,做子宫内膜细胞的检测.结果:A、B两组腹腔冲洗液中子宫内膜细胞阳性率分别为47.1%(33/70)、81.4%(57/70),术前(对照组)腹腔冲洗液子宫内膜细胞阳性率为24.3%(34/140);A、B两者比较、对照组与A、B两组比较差异有统计学意义.结论:宫腔镜手术能够造成子宫内膜细胞的腹腔播散;宫腔镜手术中子宫内膜细胞的播散与膨宫压力大小有关.%Objective:To study the endometrial cell dissenination in different distending uterine cavity pressure and to observe the safety in the hysteroscopy.Methods: 140 patients were obtained under laparoscopic joint surgery patients, randomly be divided into group A and group B,each group of 70 cases,the distending uterine cavity pressure of group A is 80 to t20 mmHg,and group B is 120 to 160 mmHg,At the same time the peritoneal washing was collected and the cytologial examinations were formed.Results:A,B two groups in peritoneal fluid cytologyrespectively for 47.1％(33/70),81.4％(57/70),peritoneal fluid cytology rate of preoperative is 24.3％ (34/140);A,B both comparison,control group and A and B two groups of comparisons difference was statistically significant.Conclusion:Hysteroscopy surgery can cause the spread of endometrial cells, The rate of the endometrial cell dissenination increased with the increasing of distending uterine cavity pressure.
