痔上黏膜环切术(PPH)联合内括约肌切开术治疗产后妇女混合痔伴肛裂34例

摘要

目的:探讨痔上黏膜环切术(PPH)加肛门后正中内括约肌切开术治疗产后妇女混合痔伴肛裂的临床疗效.方法:回顾分析我院(2005～2009)年对34例混合痔合并有肛裂的产后妇女患者行内括约肌切开术联合PPH治疗的临床资料.结果:34例行PPH加肛门后正中内括约肌切开术患者术后疼痛比单纯传统复合手术明显缓解,痣核脱出、嵌顿、出血、便秘等症状均消失,术后肛缘平整,随访(3～12)个月,痔无复发,无肛门狭窄、大便失禁等.结论:对于混合痔同时合并有肛裂的产后妇女,也可采用PPH联合肛门后正中内括约肌切开术治疗,有利于扩大PPH适应证,提高患者满意度.%Objective:To investigate the mucosal resection of hemorrhoids(PPH)after the median increase in anal sphincterotomy mixed hemorrhoids treatment of postpartum women with anal fissure therapy.Methods:We reviewed our hospital from 2005 to 2009,34 cases of mixed hemorrhoids complicated with fissure in patients with postpartum women sphincterotomy combined PPH clinical data.Results:After 34 routine PPH median increase in anal sphincterotomy alone in patients with postoperative pain than traditional surgery significantly ease the complex.hemorrhoids prolapse,incarceration, bleeding.constipation and other symptoms disappeared,after anal margin formation,followed by(3～12)months,hemorrhoids no recurrence,no anal stenosis,fecal incontinence.Conclusion:Mixed concomitant anal fissure postpartum women,also can be used within PPH joint anal aphincterotomy after the treatment center will help expand the PPH indications,increase patient satisfaction.
