银涂层导尿管对降低导尿管相关的泌尿道感染的作用

         

摘要

Objective:To observe the novel (silver coated) catheter and traditional silicone catheter to effect of urinary infection. Methods:We prospectively enrolled 100 patients and were randomly divided into novel catheter group and traditional Foley catheter group,the median duration of indwelling catheter were 9 days.The patient urine specimen were taken for the determination of urinary cells and bacterial culture.The bacteria were culturing and classification after the catheter removed to use catheter distal end.Results:The bacteriuria occurred significantly delay in patients to use of silver coated catheter( x 2=8.9;P<0.003).Kaplan Meier survival curves analysis to show the bacteriuria was time- dependent (P<0.05).Silver coated catheter group bacteriuria occurred in 11 cases (22.4%),and silicone catheter group in 33 cases (62.3%).Among 42 isolated microbial samples,gram negative bacteria was 22 (the most was Escherichia coli Enterococcus was 9,the Streptococcus and Staphylococcus aureus was 5,positive Pseudomonas was 3 as well as the 3 cultures a variety of microbiological quality system.Two groups of microbial strains was not significantly different ( relative to the number),but the control group of Pseudomonas and Staphylococcus occurred slightly higher.Before catheterization,and 7,14,21 d after catheterization silica catheter group urine positive cells were l,4,7,9;silver coated catheters group were 0,l,2,5(P<0.05).Conclusion:Novel catheter mey reduce the catheter associated urinary tract infections.%目的:观察新型(涂银)导尿管与传统硅胶导尿管对尿管相关的泌尿系感染的影响.方法:前瞻性研究设计,100例病人随机分成新型导尿管组和传统导尿管组,尿管留置时间中位数为9天,留取尿样测定尿白细胞、行细菌培养,拔除尿管后,尿管末端行细菌培养及细菌分类.结果:使用银涂层导尿管菌尿的发生明显延迟(χ2=8.9;P<0.003).Kaplan Meier生存曲线显示菌尿的发展呈时间依赖性(P<0.05).银合金导尿管组菌尿的发生为11例(22.4%),硅胶导尿管组为33例(62.3%).42个分离到微生物的样本中,22个为革兰氏阴性菌(大多数为大肠杆菌)阳性,9个为肠球菌,5个为链球菌和葡萄球菌,3个为假单胞菌阳性以及3个培养物显示多种微生物品系.总的来看,两组间微生物的品系没有明显差别(相对数量),但对照组假单胞菌和葡萄球菌的发生稍偏高.导尿前、导尿后7,14,21天硅胶尿管组尿白细胞阳性数分别为1,4,7,9;银合金导尿管组分别为0,1,2,5(P<0.05).结论:新型导尿管对减少导尿管相关泌尿道感染有一定作用.
