硝苯地平联合甘露醇对高血压脑出血患者脑循环血流动力学参数的影响

摘要

Objective:To investigate the impact of hemodynamic parameters in patients with hypertensive cerebral hemorrhage treated by Nifedipine and Mannitol.Methods:120 patients with hypertensive intracerebral hemorrhage from November 2010 to August 2012 were randomly divided into the study group and the control group. The control group were given Mannitol and the study group were given Mannitol and nifedipine. The changes of the hemodynamic parameters of the two groups were investigated. Results:The average carotid blood flow velocity and peripheral resistance, critical pressure oh the study group improved significantly. The brain tissue hematoma volume of the study group was significantly decreased (P<0.05). There was statistically difference of the overall improvement rate between the two groups.Conclusion: Nifedipine combined with Mannitol has a better advantage in the treatment of hypertensive intracerebral hemorrhage and plays an important role in improving cerebral hemodynamic parameters.%目的：探讨硝苯地平和甘露醇对高血压脑出血患者脑循环血流动力学参数的影响。方法：选取我院神经内科2010年11月~2012年8月收治的经颅CT或MRI确诊为高血压脑出血的患者120例，随机分为研究组和对照组，对照组给予甘露醇常规治疗，研究组在对照组的基础上加用硝苯地平，考察两组患者治疗后脑循环血流动力学参数的变化。结果：研究组颈动脉平均血流速度和外周阻力、临界压力明显改善，研究组脑组织血肿量明显下降(P<0.05)，研究组整体好转率与对照组比较差异有统计学意义(P<0.05)。结论：硝苯地平联合甘露醇治疗高血压脑出血具有较好的优势，提示硝苯地平在改善脑循环血流动力学参数方面起着重要的作用。
