纤支镜下大剂量沐舒坦灌洗治疗高血压重度脑出血后肺部感染

[ABSTRACT]Objective:To investigate the Effect of larger dose of ambroxol combined with bronchoalveolar lavage on pulmonary infection of tracheostomy patients after Severe cerebral hemorrhage. Methods: From August 2012 to June 2013, Fifty-six patients were chosen in our study and divided randomly into combination therapy group (larger dose of ambroxol combined with bronchoalveolar lavage,n=28) and control group(bronchoalveolar lavage alone,n=28). the pulmonary infect were described by Clinical Pulmonary Infection Score. Results: Clinical Pulmonary Infection Score in the combination therapy group was significantly reduced since the 5th and 7th d(P<0.05) . Conclusion:Larger dose of ambroxol combined with bronchoalveolar lavage has more obvious therapeutic effect on pulmonary infection than bronchoalveolar lavage alone.%目的：：观察纤维支气管镜联合大剂量沐舒坦肺灌洗对重度脑出血合并肺部感染的疗效。方法：2012年8月至2013年6月，应用纤维支气管镜下灌洗治疗重度脑出血合并肺部感染患者56例。按随机数字表法将其随机分为两组：实验组28例，沐舒坦90mg加生理盐水100ml，支气管肺泡灌洗。对照组28例，生理盐水100ml灌洗。气管切开术后第1、3、5、7天评估患者肺部感染评分。结果：气管切开术后第5、7天联合治疗组评分较对照组明显下降，差异均有统计学意义（P＜0.05）。结论：大剂量沐舒坦支气管肺灌洗较单纯生理盐水灌洗治疗重度高血压脑出血合并肺部感染有较好的临床疗效。
