2型糖尿病周围神经损害与膝关节骨关节炎严重程度的关系研究

         

摘要

目的:探讨2型糖尿病(T2DM)并发周围神经损害与膝关节炎病变严重程度的相关性.方法:回顾性分析我院风湿内分泌科2009年至2013年收治的107例T2DM合并膝骨关节炎患者的临床资料,根据是否并发糖尿病周围神经损害分为周围神经损害组43例、周围神经正常组64例,比较两组患者的膝关节骨性关节炎病变严重程度及相关指标的差异.结果:周围神经损害组的膝关节分级中,1级、2级分布比例低于周围神经正常组,3级、4级分别比例高于周围神经正常组(Z=-3.166,P=0.002),将周围神经损害定义为2级、周围神经正常定义2级,采用Spearman检验分析周围神经损害与膝关节骨性关节炎的病变程度相关性得到两者呈显著的正相关关系(r=0.307,P=0.001)周围神经损害组患者的IL-6、TNF-a、sIL-4R、OPN、CRP值均显著的高于周围神经正常组(P<0.05).结论:T2DM并发周围神经损害与膝关节骨性关节炎病变程度有一定的关系,由于周围神经的损害导致骨性关节炎早期患者感觉减退可能进一步造成疾病病情加重.%Objective: To explore the relationship between type 2 diabetes mellitus (T2DM) and peripheral nerve injury in patients with knee osteoarthritis severity.Methods:Clinical data were retrospectively analyzed in our hospitalfrom 2009 to 2013 were rheumatic department of endocrinology of 107 T2DM patients with knee osteoarthritis patients,according to whether theperipheral nerve damage in patients with diabetes were divided into peripheral nerve injury group and 43 cases of peripheral nerve,64 cases of normal group,the difference between the two groups of patients with knee osteoarthritis severity and related index.Results:Knee joint classification of peripheral nerve injury group,1 level,2 level distribution ratio is lower than theperipheral nerve in normal group,level 3,level 4respectively higher than the proportion of peripheral nerve in normal group (Z=-3.166,P=0.002),theperipheral nerve damage is defined as a level 2,peripheral nerve normal defines 2 levels of extent of lesion by Spearman test,correlation analysis peripheral nerve injury and osteoarthritis of the knee are the significant positive correlation was found between the two(r=0.307,P=0.001).Peripheral nerve damage in patients with IL-6,TNF-a group,sIL-4R,OPN,CRP values were significantly higher than normal group(P < 0.05) of peripheral nerve.Conclusion:There is a certain relationship between peripheral nerve injury of T2DM concurrent with knee osteoarthritisseverity,osteoarthritis in patients with early hypoesthesiamay further result in disease exacerbations due toperipheral nerve damage cause.
