瞬时波强技术对系统性硬化病患者心血管系统功能的评价

摘要

Objective To investigate the characteristics of wave intensity( WI ) curve for systemic sclerosis( SSc )patients. Methods Twenty-nine healthy subjects in control group and 24 SSc patients in SSc group were included in this study. And the two groups were well-matched in age, sex, heart rate and blood pressure. Left ventricular ejection fraction ( LVEF ) and fractional shortening ( LVFS ) were detected by conventional echocardiography for all subjects at resting condition. The right and left common carotid artery were targeted for ultrasonic imaging to obtain WI parameters including instantaneous accelerating wave intensity ( W1 ),instantaneous deceleration wave intensity( W2 ),negative area( NA ),the pre-eject time( R-W1 )and the eject time( W1 - W2 ). Among them,R-W1 and W1 - W2 were adjusted with R-R time. Moreover the data of stiffness parameter( (3 ), pressure-strain elastic modulus( Ep ), arterial compliance( AC ), augmentation index ( AI )and WI pulse wave velocity( PWVWI )were also recorded. Each parameter was compared between the two groups. And the correlation of LVEF and W1 was analyzed. Results In SSc group W1 and NA were ( 7.00 ±3. 14 ) × 103( mm Hg · m/s3 ) and ( 26. 20 ± 14. 35 )mm Hg · m/s2 ,while they were( 7.48 ±3. 16 ) × 10 ( mm Hg · m/s )and( 22. 39 ± 10. 86 )mm Hg · m/s in control group respectively. There was no significant difference between the two groups for W1 and NA( ( = -0. 763 and 1. 543 ,both P >0. 05 ). AI and PWVWI were( 11. 14 ±4. 88 )% and 6. 23 ± 1. 98 )m/s in SSc group, while they were( 14. 43 ±7. 67 )% and ( 5. 36 ± 1. 95 )m/s in control group respectively. AI in SSc group was lower than that in control group ( t = - 2. 526,P <0.05 ). However,PWVWI in SSc group was higher than that in control group( t = 2. 164,P < 0. 05 ). There were significantly positive correlations of W, and LVEF in both groups( r = 0. 675 and 0. 608 ,both P< 0.01 ). Conclusions The carotid elasticity may have decreased in patients with SSc before the obvious alterations in LVEF. WI could be a noninvasive method of assessing the cardiovascular function for the patients with SSc.%目的 探讨系统性硬化病(SSc)患者瞬时波强(WI)曲线特征.方法 SSc患者(SSc组)24例,健康对照组29例,两组年龄、性别、心率及血压相匹配,均于静息状态下行常规超声心动图检查,测量左心室射血分数(LVEF)及短轴缩短率(LVFS).应用WI技术测量左右颈总动脉瞬时加速度波强W1,瞬时减速度波强W2,负向波面积(NA),射血前期时间R-W1,射血时间W1-W2,并对时间分别以R-R间期校正.同时记录血管僵硬度(β)、压力应变弹性系数(Eρ)、顺应性(AC)、膨大指数(AI)、WI脉搏波传导速度(PWVWI)等血管弹性指标.将两组的各参数进行比较及对LVEF与W1进行相关性分析.结果 SSc组W1与NA分别为(7.00±3.14)×103(mm Hgm/s3)、(26.20±14.35)mm Hgm/s2,健康对照组分别为(7.48±3.16)×103(mm Hgm/s3)、(22.39±10.86)mm Hgm/s2,两组比较差异均无统计学意义(t=-0.763、1.543,P均>0.05);SSc组AI及PWVWI分别为(11.14±4.88)%、(6.23±1.98)m/s,健康对照组分别为(14.43±7.67)%、(5.36±1.95)m/s,两组比较差异有统计学意义(t=-2.526、2.164,P均<0.05);SSc组和健康对照组的W1分别与相应的LVEF呈显著正相关(r=0.675,0.608,P均<0.01).结论 SSc患者颈动脉弹性较正常人降低,而左心功能无明显损害.WI可作为评价SSc患者心血管系统功能的无创检查方法.
