正常人群左心房容积指数与二尖瓣舒张早期血流峰值和组织多普勒舒张早期峰值比值的研究

     

摘要

Objective To assess the distribution of left atrial volume indication(LAVI) for echocar-diogrphy and the early mitral diastolic flow peak value(E) and mitral tissue Doppler early dias-tolic peak value(EM) ratio in normal people and their relation. Methods A total of 650 healthy individuals were randomly divided into 24 - 30 years old group (w = 34) , 31 - 40 years old group (w = 59),41 - 50 years old group(rc = 107) , 51 - 60 years old group(w= 172) , 61 - 70 years old group(ra=131) ,71 - 80 years old group(?7 = 128) ,and 81 - 90 years old group(n= 19) according to their age. Ratio of left ventricular diastolic function indication(E) and mitral diastolic flow peak . Value (A) ,and Em,E/Em, LAV, LA VI were measured by 2D echocardiography and tissue Doppler examination,respectively,with their change observed in different age groups. Results The LAV, LAVI,and E/Em tended to increase while the E/A and Em tended to decrease in an age-dependent manner(P<0. 001). The LAVI was positively related with the E/Em in all subjects(r = 0. 212,P<0. 01) and with the LAV and E/Em in 81 - 90 years old group(r = 0. 682,P<0. 01;r= 0. 560,P<0. 05). Conclusion The LAV,LAVI,and E/Em increase while the E/A and Em decrease in normal people in an age-dependent manner. Combined LAVI and E/Em contributes better to the assessment of left ventricular diastolic function in people with their age 2>81 years.%目的 评价超声心动图指标左心房容积(left atrial volume,LAV)指数(LAVI)与反映左心室充盈压的二尖瓣舒张早期血流峰值(E)和组织多普勒二尖瓣环舒张早期峰值速度(Em)比值在正常人群中的分布及关系.方法 随机调查正常人群650例.按年龄分为7组,24~30岁组34例,31~40岁组59例,41~50岁组107例,51~60岁组172例,61~70岁组131例,71~80岁组128例,81~90岁组19例.用二维超声及组织多普勒的方法检测左心室舒张功能指标E与左心室二尖瓣舒张晚期血流峰值(A)的比值、Em、E/Em、LAV、LAVI,观察上述指标在不同年龄组的变化.结果 各组随年龄增长,LAV、LAVI、E/Em呈增高趋势,而E/A、Em呈降低趋势,差异有统计学意义(P<0.01).整体人群LAVI与E/Em呈正相关(r=0.212,P<0.01).81~90岁组LAVI和LAV与E/Em呈正相关(r=0.682,P<0.01;r=0.560,P<0.05).结论 正常人群LAV、LAVI、E/Em随年龄增长呈增高趋势,E/A、Em随年龄增长呈降低趋势.年龄≥81岁人群应用LAVI与E/Em相结合评价左心室舒张功能意义更大.
