CT扫描联合磁共振诊断原发性肝癌及评估其介入治疗术后的临床效果

         

摘要

目的 探讨CT扫描联合磁共振(magnetic resonance imaging,MRI)在原发性肝癌的诊断及介入治疗术后的临床评估中的应用价值.方法 回顾性分析166例原发性肝癌患者的临床资料,CT与MRI联合应用的75例患者为联合组,只做MRI常规检查的91例患者为MRI组,比较两组患者的影像学表现、术前诊断率及介入治疗术后的疗效评估.结果 联合组术前诊断率(96.0%)高于MRI组(89.0%),差异有统计学意义(P<0.05).在临床疗效评估上,最终随访130例患者,共142个肿瘤病灶.2名阅片医师一致性检验,Kappa值为0.89,具有良好一致性;两种影像学方法判断残留强化区的Kappa检验分析,Kappa值为0.50,一致性好.两组残留动脉期强化区最大长径平均值差异无统计学意义(P=0.065),两组最小短径平均值比较,差异有统计学意义(P=0.034).结论 CT扫描联合MRI在原性发肝癌的诊断及介入治疗术后的临床效果评估中能起到良好的互补作用,具有一定的临床应用价值.%Objective To investigate the clinical effect of CT scanning combined with magnetic resonance imaging (MRI) in diagnosis and interventional treatment for postoperative analysis of primary liver cancer.Methods The clinical data of 166 patients with primary liver cancer were retrospectively analyzed, 75 patients given CT scan combined with MRI were as combined group, 91 patients simply given routine MRI examination were as MRI group.Imaging findings, preoperative diagnosis rate and postoperative interventional treatment curative effect were compared between two groups.Results The diagnostic rate of combined group (96.0%) was higher than MRI group (89.0%), there were statistically significant difference (P<0.05).On the clinical curative effect evaluation, 130 patients were followed-up, there were 142 cases of tumor lesions.Two reading consistency kappa value was 0.89, and had a good consistency.Two kinds of imaging methods to judge the Kappa test analysis of residual reinforcement area, Kappa value was 0.50, it had a good consistency.There was no statistical difference in average residual reinforced regions largest arterial length to diameter between two groups (P=0.065), and there was statistically significant difference in minimum average of short diameter between two groups (P=0.034).Conclusion The clinical effect of CT scan combined with MRI in the diagnosis and interventional treatment for postoperative analysis of primary liver cancer plays a good complementary role.
