原发性高血压患者血压变异性与糖尿病的相关性分析

     

摘要

目的探讨原发性高血压合并2型糖尿病(T2DM)患者与单纯原发性高血压患者动态血压水平及血压变异性的相关性.方法选择2016年1月~2018年7月于我院心内科就诊的原发性高血压合并2型糖尿病(T2DM)患者280例作为观察组,另选择同期心内科诊断为原发性高血压患者300例作为对照组.监测两组患者日间、夜间及24 h收缩压(SBP)及舒张压(DBP)变异性,并对比分析.结果观察组的24 h平均SBP、夜间平均SBP均明显高于对照组,(P<0.05),观察组SBP昼夜差值、DBP昼夜差值明显低于对照组,(P<0.05),观察组24h SBP标准差、24h日间SBP标准差、日间SBP变异系数、夜间SBP标准差、夜间SBP变异系数、夜间DBP标准差、血压晨峰均明显高于对照组(P均<0.05).结论观察组患者血压变异性较观察组增高,提示原发性高血压合并2型糖尿病时,对患者的血压昼夜调节影响较大,可能与血糖水平变化有关.%Objective To investigate the correlation between dynamic blood pressure and blood pressure variability in patients with essential hypertension complicated with type 2 diabetes mellitus (T2DM) and patients with essential hypertension.Method 280 patients with essential hypertension and type 2 diabetes mellitus (T2DM) who were admitted to our department of cardiology from January 2016 to July 2018 were selected as observation group, and300 patients with primary hypertension diagnosed in the same period.For example, it is a control group.The variability of daytime, nighttime, and 24 h systolic blood pressure (SBP) and diastolic blood pressure (DBP) were monitored and compared.Result The mean SBP and nighttime SBP of the observation group were significantly higher than those of the control group (P<0.05).The difference of SBP between day and night and DBP in the observation group was significantly lower than that of the control group (P<0.05).SBP standard deviation, 24h day SBP standard deviation, daytime SBP coefficient of variation, nocturnal SBP standard deviation, nocturnal SBP coefficient of variation, nocturnal DBP standard deviation, and blood pressure morning peak were significantly higher than the control group (P<0.05).Conclusion The blood pressure variability of the observation group was higher than that of the observation group, suggesting that primary hypertension combined with type 2 diabetes has a greater impact on the patient's blood pressure circadian regulation, which may be related to changes in blood glucose levels.
