高血压患者亚极量运动时血压变化及其影响因素

         

摘要

目的:探索原发性高血压患者亚极量运动时血压的变化及其影响因素。方法连续入选2014年12月~2015年7月在北京军区总医院就诊的原发性1、2级高血压患者(高血压组)82例和健康体检者(正常组)54例。观察高血压患者亚极量运动时血压变化,并采用多元线性回归分析运动前血压、脉压及踝臂动脉脉搏波传导速度(baPWV)等因素与亚极量运动收缩压的关系。结果与正常组相比,高血压组亚极量运动收缩压较高[(190±13.2)mmHgvs.(136.7±15.3)mmHg,(1mmHg=0.133kPa)P<0.05],上升幅度较高[(39.0±12.1)mmHgvs.(21.7±10.4)mmHg,P<0.05]。多元线性回归分析显示:运动前收缩压、baPWV及脉压与亚极量运动收缩压线性相关(P<0.05)。结论原发性高血压患者亚极量运动时血压升压反应高于正常人,其亚极量运动收缩压与运动前收缩压、baPWV及脉压紧密相关。%Objective To explore the changes of blood pressure and its influence factors in primary hypertension patients during submaximal exercise.Methods 82 patients with primary hypertension ( hypertension group) and 54 normal subjects (normal control group) were enrolled from December 2014 to July 2015. Changes of blood pressure with primary hypertension during submaximal exercise were measured. Multiple regression analysis was used to analyze relationship between systolic pressure during submaximal exercise with blood pressure before exercise, pulse pressure, and brachial-ankle pulse wave velocity (baPWV).Results Compared with normal control group, systolic pressure during submaximal exercise in hypertension group was higher (190±13.2 mmHgvs. 136.7 ±15.3 mmHg,P<0.05), with higher rising range (39.0±12.1 mmHgvs. 21.7±10.4 mmHg,P<0.05). Multiple stepwise regression analysis result showed that pre-exercise systolic blood pressure, baPWV and pulse pressure were related to systolic blood pressure during submaximal exercise (P<0.05).Conclusion In primary hypertension patients, blood pressure change during submaximal exercise is higher than normal subjects. Pre-exercise systolic blood pressure, baPWV and pulse pressure were related to systolic blood pressure during submaximal exercise.
