康复运动训练联合心理治疗对冠心病PCI术后患者的影响

摘要

Objective To compare effects between routine rehabilitation exercise training combined with psychological therapy and simple routine rehabilitation exercise training for 3 months after PCI (Percutaneous Coronary Intervention) in CHD (coronary heart disease) patients.Methods To choose the cardiovascular internal medicine, 100 CHD patients accepted primary PCI from Liaoning electric power center hospital (40~80 years old) were randomly divided into control group (routine rehabilitation exercise) and observation group (routine rehabilitation exercise training combined with psychological therapy). Each group had 50 patients. Within two weeks after PCI, intervention would be given to all of the patients for three courses. Each course contains once intervention per week and lasts for four weeks. Hamilton anxiety scale (HAMD) scores, Hamilton depression scale (HAMA) score, 6 min walking test, left ventricular ejection fraction, metabolic equivalent and their quality of life were compared. Results: After 12 weeks’ intervention, HAMD scores and HAMA scores were decreased, especially in observation group (P<0.05).Results of 6 min walking distance test, left ventricular ejection fraction and metabolic equivalent in observation group were better than control group (P<0.05). Quality of life in observation group was more improved than control group (P<0.05).Conclusion Rehabilitation exercise training with psychological therapy is important CHD patients after PCI in health improvement.%目的：探讨常规康复运动训练联合心理治疗相对单纯常规康复运动训练3个月后对冠心病行经皮冠状动脉介入治疗（PCI）术后患者的不同影响。方法选自辽宁电力中心医院心血管内科冠心病PCI术后患者100例，年龄40~80岁，采用随机方法分为对照组（进行常规康复运动）和观察组（在常规康复运动训练基础上联合心理治疗），每组各50例，两组患者均于术后2周进行干预，每周1次，4周为一个疗程，共3个疗程。观察分析两组患者汉密顿焦虑量表（HAMD）评分、汉密顿抑郁量表（HAMA）评分、6 min步行试验、左心室射血分数、代谢当量、生活质量的改变。结果两组患者干预12周后，HAMD评分、HAMA评分均下降，观察组下降显著（P＜0.05）；6 min步行试验距离、左室射血分数和代谢当量均提高，观察组均优于对照组（P＜0.05）；观察组患者生活质量明显高于对照组（P＜0.05）。结论康复运动训练联合心理治疗对冠心病PCI术后患者的身体状况的改善有重要意义。
