目的 探讨血管内支架置入治疗高龄基底动脉狭窄患者的有效性及安全性.方法 纳入2008年7月~2015年5月于广西医科大学第一附属医院和附属民族医院神经内科住院的基底动脉狭窄患者34例,其中男性25例,女性9例.依据年龄分为<75岁组(17例)和≥75岁组(17例).比较手术成功率、围手术期并发症,术后随访并记录临床终点事件.结果 两组患者各置入支架17枚,均一次性成功置入,手术成功率为100%.两组手术成功率比较差异无统计学意义(P>0.05).两组患者围手术期并发症发生率比较(17.6%vs.5.9%),差异无统计学意义(P>0.05).≥75岁组患者随访期间共2例(11.7%)发生临床终点事件,1例(5.9%)为脑梗死,1例(5.9%)为短暂性脑缺血发作.<75岁组患者也有2例(11.7%)发生临床终点事件,均为短暂性脑缺血发作.随访结束无患者死亡.结论 血管内支架置入治疗高龄基底动脉狭窄患者安全、有效,值得临床推广.%Objective To discuss the effectiveness and safety of intravascular stenting for treating basilar artery stenosis in elderly patients.Methods The patients (n=34, male 25 and female 9) were chosen from the Department of Neurology of the First Affiliated Hospital and National Hospital of Guangxi Zhuang Autonomous Region affiliated to the Guangxi Medical University from the July 2008 to May 2015. All patients were divided, according to age, into age<75 group and age≥75 group (eachn=17). The success rate of the operation and perioperative complications were compared, and clinical end-point events were followed up and recorded. Results There were 17 stents implanted successfully at once in each groups respectively, and the success rate of the operation was 100% in 2 groups (P>0.05). The difference in incidence of perioperative complications had no statistical significance between 2 groups (17.6%vs. 5.9%). During follow-up period, in age≥75 group, there were 2 (11.7%) patients with clinical end-point events, and 1 (5.9%) with cerebral infarction and 1 (5.9%) with transient ischemic attack, and in age<75 group, there were 2 (11.7%) patients with clinical end-point events, and all of them with transient ischemic attack. There were no patients died after follow-up finished.Conclusion The intravascular stent implantation is safe, effective and worth clinical promoting in treatment of senile basilar artery stenosis.
