Objective: To explore the principle and method of prescription review on perioperative antimicrobial prophylaxis in clean surgery. Methods: Based on cases of three types of clean surgery and relevant literatures from home and abroad, the prescription review strategies of perioperative antimicrobial prophylaxis in clean surgery, such as evaluating indications, drug selection of species, timing of administration, duration and other key issues were clarified. Results:The prophylactic use of antimicrobial in three kinds of clean surgery was not be recommended in our country routinely. The recommendations on antimicrobial prophylaxis in thyroidectomy were consensus-based between China and USA, while perioperative antimicrobial prophylaxis in inguinal hernia and breast cancer surgery was recommended in USA. Conclusion:Prophylactic use of antimicrobial in perioperative period of clean surgery should be based on individual patient and the differences of operating methods in surgical procedures. In order to carry out an objective prescription review on antimicrobial prophylaxis in clean surgery, pharmacists should fully assess patients' condition and progress of literature.%目的：：探讨实施清洁手术围手术期抗菌药物预防应用的处方医嘱点评的原则和方法。方法：基于3种类型的清洁手术病例，结合国内外相关文献，明确药师在围手术期抗菌药物预防应用的适应证、药物品种选择、给药时机、持续时间等问题的点评策略。结果：国内法规文件不推荐3种清洁手术在围手术期常规预防应用抗菌药物；国外文献对甲状腺切除术的推荐意见与国内一致，但对腹外疝修补术和乳腺癌切除术则推荐围手术期抗菌药物预防应用。结论：清洁手术因不同术式和患者个体差异，在围手术期抗菌药物应用上存在不同，药师应充分评估患者情况，结合文献进展，开展客观、正确的处方点评工作。
