α-硫辛酸治疗伴腹型肥胖的2型糖尿病勃起功能障碍临床研究

摘要

Objective To observe the therapeutic effects of alpha lipoic acid( LA ) in type 2 diabetes mellitus ( DM ) with abdominal obesity( AO ) and erectile dysfunction( ED ). Methods All patients involved type 2 DM were divided into two groups: the AO group ( 30 type 2 DM patients with ED and AO ) and the non AO group ( 30 type 2 DM patients with ED but without AO ). The therapeutic methods were similar of fundamental treatment plus alpha lipoic acid in both groups, and the period of therapy was 3 weeks. The comparison of results was performed before and after treatment. Results Before the LA treatment, the indexes of AUC, ISI, SOD, GSH-Px, NO, score of IIEF-5 and endothelium dependent dilation were lower in AO group than those in the non AO group ( P <0. 05 ), while the indexes of MDA and ET-1 were higher than those in the non AO group ( P < 0. 05 ), there were no significant difference in endothelium independent dilation between 2 groups ( P > 0. 05 ). After the LA treatment, the indexes of the AUC, ISI, SOD, GSH-Px, NO, score of IIEF-5 and endothelium dependent dilation increased significantly ( P < 0. 05 ), and the MDA, ET-1 decreased significantly ( P < 0. 05 ) in both two groups, which were more obvious in the AO group ( P < 0. 05 ). There was no significant difference in endothelium independent dilation before and after the LA treatment in both groups ( P > 0. 05 ). Conclusion It demonstrated that the LA could improve the beta cell function and the status of oxidative stress and endothelium function in type 2 DM patients with AO and ED, and was of good therapeutic effect for ED.%目的 观察α-硫辛酸对伴腹型肥胖的2型糖尿病勃起功能障碍患者的疗效.方法 选择伴腹型肥胖(腹型肥胖组)和不伴腹型肥胖(非腹型肥胖组)的2型糖尿病勃起功能障碍患者各30例.2组患者均予基础糖尿病治疗并加用α-硫辛酸,治疗3周后进行效果比较.结果 治疗前,腹型肥胖组AUC、ISI、SOD、GSH-Px、NO、IIEF-5评分及内皮依赖性血管舒张功能均低于非腹型肥胖组(P＜0.05),MDA、ET-1均高于非肥胖组(P＜0.05),内皮非依赖性血管舒张功能2组无明显差异(P＞0.05);治疗后,2组AUC、ISI、SOD、GSH-Px、NO、IIEF-5评分及内皮依赖性血管舒张功能均升高(P＜0.05),MDA、ET-1均下降(P＜0.05),且腹型肥胖组的改善幅度大于非腹型肥胖组(P＜0.05),2组内皮非依赖性血管舒张功能均无明显变化(P＞0.05).结论 α-硫辛酸能改善腹型肥胖2型糖尿病患者勃起功能障碍的胰岛β细胞功能、氧化应激状态和内皮功能,治疗糖尿病勃起功能障碍有良好效果.
