食管鳞癌组织中环氧化酶-2、P-糖蛋白的表达及临床意义

         

摘要

Objective To study the Cyclooxygenase -2(COX-2) and P-glycoprotein (P-gp) expression in esophageal squamous carcinoma tissues and its clinical significance .Methods The immunohistochemical method to detect COX-2 and P-gp in esophageal squamous carcinoma tissues and normal esophageal tissue expression .Results The whole set of esophageal tissue COX-2 expression positive rate was 60.0%(36/60), no expression was found in normal esophageal tissue .COX-2 ex-pression icreased as lesions infiltrating deepens , higher the degree of differentiation , lymph node metastasis and gradually in-crease (χ2 =8.140, P =0.043;χ2 =15.952, P =0.001;χ2 =13.302, P =0.001), the expression of COX-2 was associat-ed with TNM staging,III,IV expression was significantly higher in I ,II period (χ2 =17.778, P =0.001).Full set of esopha-geal tissue P-gp expression positive rate was 45.0%(27/60), normal tissue positive rate was 20.0%(12/60).The expres-sion of P-gp as the tumor infiltration depth deepening ,nodal metastasis and elevated expression (χ2 =10.649, P =0.014;χ2 =6.877, P =0.009) P-gp expression was associated with the TNM staging of esophageal cancer ,III,IV period of expression than I,II period (χ2 =5.455, P =0.020).COX-2 and P-gp in the expression of esophageal squamous carcinoma tissues were positively correlated( r =0.397, P =0.021 ).Conclusion COX-2, P-gp involved in esophageal cancer occurrence , devel-opment and transfer of multiple links .The expression of COX-2 and P-gp were positively correlated , COX-2 can regulate the expression of P-gp through some mechanism , to participate in esophageal multi-resistant adjustment .%目的:探讨环氧化酶-2(COX-2)与P-糖蛋白(P-gp)在食管鳞癌组织中的表达及其临床意义。方法选择60例食管鳞癌患者肿瘤组织与正常组织,应用免疫组织化学方法检测COX-2和P-gp在食管鳞癌组织及正常食管组织中的表达情况。结果食管癌组织COX-2表达的阳性率为60膊.0%(36/60),正常食管组织未表达。 COX-2的表达随着病变浸润的加深,分化程度的升高,淋巴结的转移而逐渐上调(χ2=8.140, P =0.043;χ2=15.952, P=0.001;χ2=13.302, P =0.001),COX-2的表达与 TNM 分期相关,Ⅲ、Ⅳ期中的表达明显高于Ⅰ、Ⅱ期(χ2=17.778, P =0.001)。食管癌组织P-gp表达的阳性率为45.0%(27/60),正常组织阳性率为20.0%(12/60),差异有统计学意义( P <0.01)。 P-gp的表达随着肿瘤浸润深度的加深,淋巴结的转移而表达升高(χ2=10.649, P =0.014;χ2=6.877, P =0.009)。 P-gp的表达与食管癌的TNM分期相关,Ⅲ+Ⅳ期中的表达高于Ⅰ+Ⅱ期(χ2=5.455, P =0.020)。 COX-2与P-gp在食管鳞癌组织的表达呈正相关( r =0.397, P =0.021)。结论 COX-2、P-gp参与了食管癌发生、发展、转移的多个环节,COX-2可以通过某种机制调控P-gp的表达,从而参与食管癌多药耐药的调节。
