妊娠糖尿病患者血清FABP4、chemerin、Nesfatin-1与胰岛素抵抗和胰岛β细胞功能的关系分析

     

摘要

Objective To analyze the relationship between fatty acid binding protein 4 ( FABP4 ) , chemokine (chemerin), and dietary inhibitory factor (Nesfatin-1) and insulin resistance and beta cell function in patients with gestation-al diabetes mellitus (GDM).Methods From June 2013 to October 2015, 50 GDM patients (GDM group) admitted to the first people's Hospital of Changshou City of Jiangsu province Department of Endocrinology were enrolled as the research object , the other 40 cases of healthy pregnant women as control group , 2 groups of serum FABP4,chemerin, Nesfatin-1 levels and IR and beta cell function index were compared .Results The levels of FABP4, chemerin and Nesfatin-1 in the GDM group were significantly higher than those in the healthy control group [(35.11 ±11.32) μg /L vs.(21.14 ±8.75) μg/L, (8.41 ± 3.33) mg/L vs.(2.86 ±0.74) mg/L, (4.99 ±1.61) μg/L vs.(2.01 ±0.44) μg/L, t =2.559, t =2.224, t =2.112, P =0.008, P =0.017, P =0.021].The levels of HOMA-IR, HOMA-β, INSAUC /PGAUC and△I30 /△G30 in GDM group were lower than those in healthy control group [(2.12 ±0.66) vs.(3.74 ±1.29), (165.32 ±24.45) vs.(102.27 ± 24.12) ,(10.81 ±3.95) vs.(8.01 ±3.32),(21.51 ±4.98) vs.(15.91 ±6.33), t =2.056,2.840,2.392,2.616, P =0.024, P =0.004, P =0.011, P =0.007].FABP4, chemerin and Nesfatin-1 were positively correlated with HOMA-IR (r =0.422, r =0.379, r =0.345, P <0.05), and negatively correlated with HOMA-β( r =-0.466, r =-0.312, r =-0.326, P <0.05).Conclusion The levels of FABP4, chemerin, Nesfatin-1 were significantly increased in patients with GDM, and there was some correlation between IR and pancreatic beta cell function .%目的:分析脂肪酸结合蛋白4(FABP4)、趋化素(chemerin)、摄食抑制因子1(Nesfatin-1)与妊娠糖尿病( GDM)患者胰岛素抵抗和胰岛β细胞功能的关系。方法选取2013年6月—2015年10月江苏省常熟市第一人民医院内分泌科收治GDM患者50例作为研究对象( GDM组),另选择健康孕妇40例作为健康对照组,检测2组血清FABP4、chemerin、Nesfatin-1水平及其与IR和胰岛β细胞功能的相关性。结果 GDM组患者FABP4、chemerin、Nesfa-tin-1水平均明显高于健康对照组[(35.11±11.32)μg/L vs.(21.14±8.75)μg/L,(8.41±3.33) mg/L vs.(2.86±0.74)mg/L,(4.99±1.61)μg/L vs.(2.01±0.44)μg/L, t =2.559、2.224、2.112, P =0.008、0.017、0.021]。 GDM组HOMA-IR、HOMA-β、INSAUC/PGAUC和△I30/△G30均低于健康对照组[(2.12±0.66)vs.(3.74±1.29),(165.32±24.45)vs.(102.27±24.12),(10.81±3.95,)vs.(8.01±3.32),(21.51±4.98)vs.(15.91±6.33), t =2.056、2.840、2.392、2.616, P =0.024、0.004、0.011、0.007]。 FABP4、chemerin、Nesfatin-1与 HOMA-IR 呈正相关( r =0.422、0.379、0.345, P <0.05),与HOMA-β呈负相关( r =-0.466、-0.312、-0.326, P <0.05)。结论 GDM患者体内FABP4、chemerin、Nesfatin-1水平明显上升,与IR和胰岛β细胞功能存在一定的相关性。
