ABCC4基因在胃癌组织中的表达及耐药性研究

摘要

目的 研究ABCC4在胃癌组织中的表达情况及其对化疗药物耐药性的影响.方法 2011年1月—2013年12月湖南中医药大学第一附属医院外科诊治胃癌患者115例,采用免疫组织化学方法检测ABCC4基因的表达;取部分切除的胃癌组织样本,培养待检测细胞,在体外用MTT比色法检测细胞对抗癌药物(顺铂和5-氟尿嘧啶)的敏感性,随访并分析不同ABCC4基因表达组耐药性、胃癌复发率、复发时间的差异.结果 ABCC4基因在胃癌组织中的阳性表达率为46.96%(54/115),MTT比色法检测表明,阳性表达组胃癌组织对顺铂和5-氟尿嘧啶的敏感度显著低于阴性表达组(t=9.14、10.49,P<0.01).随访结果显示,115例胃癌患者1年和3年内的生存率分别为86.1%和58.3%.ABCC4阳性表达组和阴性表达组比较,术后复发率1年、3年生存率比较差异均无统计学意义(P>0.05),但复发时间阳性表达组早于阴性表达组(P<0.05).不同ABCC4表达患者接受化疗与未接受化疗生存率差异无统计学意义(P>0.05).结论 ABCC4基因表达与胃癌细胞对抗癌药物的耐药性具有一定的关系,应该作为临床患者化疗有效性的考虑因素之一.%Objective To explore the expression of ATP binding cassette, subfamily C member 4 (ABCC4) in gas-tric cancer tissue and the effect of this gene on chemotherapeutic drug resistance of gastric cancer. Methods The expression of ABCC4 gene was detected by immunohistochemical method in 115 patients with gastric cancer who were treated at surgery department in the first hospital of Hunan university of Traditional Chinese Medicine between Janurary 2011 to December 2013. The cell culture of gastric cancer tissue was performed, and the sensitivity of the patient' s tumor cells to anti-cancer drugs ( Cisplatin and 5 fluorouracil) was measured by MTT assay in vitro. Combined with the patients' follow-up, analyze the rela-tionship between ABCC4 gene expression and drug resistance, recurrence rate and recurrence time of gastric cancer. Results The positive expression rate of ABCC4 gene in gastric cancer tissues was 46. 96%. MTT assay showed that the sensitivity of gastric cancer tissue to chemotherapeutic drugs in the positive expression group was significant lower than that of negative ex-pression group (P<0. 05). The results of patients' follow-up showed that the effect of chemotherapy was better in negative expression group than positive expression group, but the difference was not significant (P> 0. 05). Conclusion The ex-pression of ABCC4 gene have a certain relationship in the resistance of gastric cancer cells to anti-cancer drugs, and it should be one of the factors to consider the efficacy of chemotherapy in clinical patients.
