声触诊组织成像和定量技术与核磁共振诊断多发性肌炎／皮肌炎的比较

摘要

目的：对比分析声触诊组织成像和定量（VTIQ）技术与核磁共振（MRI）在诊断多发性肌炎（PM）／皮肌炎（DM）中的价值。方法：选择经病理确诊的PM／DM患者26例，健康体检者60例。应用常规超声结合 V T IQ技术分析肌肉组织的特征。VTIQ以横向剪切波速度（SWV ）值表示肌肉的弹性值，获取 SWV 最大值（SWVmax ）、最小值（SWVmin ）和平均值（SWVmean ）。应用受试者操作特征曲线（ROC）确定诊断价值最大的SWV值。计算VTIQ和MRI两种方法诊断的敏感度、特异度并比较。结果：PM／DM 患者SWVmax 、SWVmin和SWVmean均高于对照组，差异有统计学意义（P＜0．05）。ROC曲线显示SWVmax曲线下面积（AUC）最大（0．97）；以SWVmax ≥2．68 m／s作为VTIQ诊断PM／DM的指标，其敏感度为76．9％、特异度为93．3％。M RI诊断敏感度为80．7％、特异度为91．7％。两种影像学方法诊断PM／DM的A UC分别为0．851和0．854。结论：V T IQ技术对PM／DM的诊断效能与M RI相近。%Objective: To compare and contrast the clinical value of virtual touch tissue imaging quantification (VTIQ) and magnetic resonance imaging (MRI) in the diagnosis of polymyositis/dermatomyositis (PM/DM ) .Methods:Twenty‐six patients who were diagnosed with polymyositis or dermatomyositis by pathological examination and sixty healthy volunteers were included .Conventional ultrasound combined with virtual touch tissue imaging quantification was used to analyze the sonographic characteristics of muscle tissue .The values of transverse shear wave velocity (SWV ) were used to assess the sarcous flexibility ,and the maximum ,minimum ,and mean values of SWV were obtained .The group with maximum diagnostic value was determined by receiver operating characteristic curve (ROC) .The sensitivity and specificity of VITQ and MRI were calculated and compared .Results:The values of SWVmax ,SWVmin ,SWVmean in the patients with PM/DM were all higher than the control group ,and the differences were statistically significant (P<0 .05) .The area under the curve(A UC) of SWVmax was the largest (0 .97) .The cutoff value was obtained ,and being more than or equal to the cut‐off value was used as an indicator of the diagnosis of PM/DM by VTIQ .VTIQ diagnostic sensitivity was 76 .9% and specificity was 93 .3% .MRI diagnostic sensitivity was 80 .7% and specificity was 91 .7% .The AUC of the two imaging methods for the diagnosis of PM/DM were 0 .851 and 0 .854 ,respectively .Conclusions:The diagnostic efficacy of VTIQ on PM/DM is similar to MRI ,so it has some clinical value for the diagnosis of PM/DM .
