Objective: To evaluate the efficacy and safety of tirofiban hydrochloride, a specific inhibitor of platelet glyco-protein Ⅱ b/Ⅲ a receptor, in treatment of patients with acute coronary syndrome (ACS) complicated diabetes mellitus (DM) undergoing percutaneous coronary intervention (PCI). Methods: A total of 160 ACS patients complicated DM received PCI were enrolled and randomly divided into two groups: routine treatment group (n=80) ; tirofiban group [n = 80, received tirofiban (made in China) based on routine treatment]. Results: Baseline clinical data and PCI data of the two groups had no significant difference. Incidence rates of major adverse cardiovascular events (MACE) on 24h and 30d after PCI in tirofiban group were significantly lower than those of routine treatment group (24 h: 5. 0％ vs. 27. 5％ , P=0. 023; 30 d: 2. 5％ vs. 22. 5％ , P = 0. 036). No severe bleeding happened in the two groups and there was no significant difference between the two groups in complication of mild bleeding (P = 0. 297). During follow-up period (30d～180d), there was no significant difference in incidence rate of MACE between the two groups (P = 0. 672). Conclusion: The study results indicate that tirofiban hydrochloride could safely and effectively reduce the incidence of MACE without significant increase in bleeding in ACS patients complicated DM undergoing PCI.%目的:观察国产血小板糖蛋白Ⅱb/Ⅲa受体拮抗剂盐酸替罗非班在治疗急性冠脉综合征(ACS)合并糖尿病(DM)患者经皮冠状动脉支架植入术(PCI)中的有效性和安全性.方法:选择2008年8月至2010年8月于我院行介入治疗的DM合并ACS患者160例,随机分为:常规治疗组80例,PCI时常规应用阿司匹林、氯吡格雷及抗凝治疗;替罗非班组80例:在常规治疗的基础上合并应用替罗非班.比较两组患者术后24h及30d,180d的主要不良心血管事件(MACE)及出血发生率.结果:两组患者基础情况及冠脉介入情况无明显差异.替罗非班组24 h、30 d的MACE发生率明显低于常规治疗组(24 h:5.0%∶27.5%,P=0.023; 30 d:2.5%∶22.5%,P=0.036),两组均未发生严重出血,轻微出血并发症在两组间无明显差异(P=0.297).在30 d至180 d的随访中两组患者的MACE发生率无显著性差异(P=0.672).结论:国产盐酸替罗非班在急性冠脉综合征合并糖尿病患者的支架植入术中使用,能进一步减少24 h、30 d的主要不良心血管事件,出血事件没有显著增加,是安全和有效的.
